It’s officially sweater weather which makes it the perfect season for lower face and body contouring treatments. Last week I shared a little laser tip while treating this patient, and this week I want to talk to all my injector friends about something to be very conscientious of when placing PDO barbed lifting threads in the submalar area. It has been noted that forceful injectors, even when using a cannula, can accidentally push through the mucosa. This is an area that requires patience and precision. Make sure you are in the correct plane and take your time as you work down into the cheek area.

While this wouldn’t necessarily cause major damage to the patient, it does create an opportunity for infection. If you are also treating with neuromodulator on the same day and the patient ends up needing antibiotics, that can lead to a variety of avoidable complications.

The key takeaway is to slow down, stay mindful of your depth, and move with intention when working with PDO threads. I have a Patreon episode coming soon that covers this full treatment in depth. There’s been a lot of chatter online about PDGF. In my personal clinic, I did a six-month trial on twelve patients, and we are absolutely obsessed with the results. We’ve seen phenomenal volume improvement, and I’ve come to love combining it with PRF Gel for enhanced skin quality.

The current conversation seems to focus on possible long-term numbness, lasting up to six months, as well as nodularity formation. I truly believe these complications occur when injections are performed in the wrong plane or when too much product is deposited in one area. It takes a truly skilled and educated approach, with a strong understanding of anatomy and the science of the product, to make safe and effective decisions when working with PDGF.

This product is not FDA-approved for aesthetic treatments. However, it is FDA-approved for wound healing and bone regeneration. I still enjoy discussing it because I believe that as more aesthetic providers become involved in research and clinical studies, we’ll begin to see more green flags and less red tape.

When treating the lower face with neuromodulators, precision is everything. The DAO and Mentalis work closely with the DLI, which pulls the lower lip down. If the neurotoxin drifts into the DLI, it can cause an uneven smile or lower lip droop, and that is not what we want.

That is why I take a strategic, low-unit approach targeting the exact fibers that need relaxing while leaving the DLI untouched. The result is lifted corners of the mouth, softened chin texture, and natural expression without compromising your smile.

I had a first time patient come in for toxin today and I have to say, he had phenomenal skin. You could tell he takes great care of it, wears his sunscreen every day, and really understands the importance of prevention. His main concern was deep set lines and a little overexpression through the forehead and glabella.

When assessing his anatomy, I noticed strong corrugators and a defined procerus, so placement and depth were really important here. I treated all the way across his forehead with small, precision driven injections to relax the head of the corrugator while keeping his movement natural. I also placed one deep unit into the procerus to prevent that little bulge that can happen as the muscle relaxes.

He’ll be back in three weeks for his follow up and we’ll share his before and afters then.

Have you ever had that annoying little eyelid twitch that seems to come out of nowhere? It’s called a blepharospasm, and while most people experience it at some point, for others it can linger for weeks or even months.

A blepharospasm is an involuntary muscle contraction that causes the eyelid to twitch, either on the upper or lower lid. It can be triggered by things like fatigue, stress, or even too much caffeine.

This particular patient experiences it consistently, so we treat it with a very small amount of neuromodulator to relax the overactive muscle and prevent that repetitive twitching. Fun fact, neuromodulators were originally approved for therapeutic uses like this long before they became popular for aesthetic treatments.

I love that these treatments can go beyond beauty and truly enhance quality of life. This is a perfect example of holistic care that brings both comfort and confidence.
