Apostolic Christian Skylines Home Care Services is rooted in the caring tradition of Apostolic Christian Skylines, an Apostolic Christian retirement community founded in Peoria, Illinois on February 12, 1912. In its present location at 7023 NE Skyline Drive since 1966, Apostolic Christian Skylines offers older adults a complete continuum of senior living that includes independent living, assisted living, memory care, skilled nursing care and skilled rehabilitation services.
In 2010, the Skylines caring tradition was extended to Peoria area older adults in their home settings with the establishment of Apostolic Christian Skylines Home Care Services as a provider of home and community-based services. In the Skylines spirit and tradition, we offer a full spectrum of compassionate, caring home care services that include companion and homemaker care, personal care assistance, home health aide services, skilled nursing services and case management.
At Apostolic Christian Skylines Home Care, the caring tradition continues.

We employ qualified workers with excellent work histories and verifiable quality references. Our caregivers provide compassionate, supportive services that assist older adults and their families in maintaining their desired environment.
All of ACS Home Care Services’ caregivers are:
• Carefully screened, including a fingerprint background check

We do regular in-home supervisory visits to ensure the client’s needs are being met. All care is overseen by a Registered Nurse.
Every caregiver completes a thorough initial training focused on the skills required for your specific needs and receives additional ongoing continuing education and training.
INITIAL ASSESSMENT
Before beginning services, ACS Home Care Services always completes an initial in-home interview, needs assessment and safety check to evaluate the level of care that is required to achieve your goals and quality of life. Using this information, a plan of care is developed
and the most appropriate caregiver identified, so you can be sure of the best care possible.
ACS Home Care Services assists people of many ages, in many different situations. We serve:

• Adults of any age following hospitalization or surgery, when walking, standing or driving may be difficult
• People with disabilities
• Hospice patients
• Assisted living residents who may need more assistance, whether temporary or long term
