Join us at Antrim Medical Group on Monday, January 12, or Friday, January 23, from 8:00 am to 4:00 pm. Free forms, staff guidance, and notary services will be available, with no registration required.

An Advance Directive helps ensure your healthcare wishes are known if you are ever unable to speak for yourself. It is never too soon to be prepared.
