MEET DANIELLE YOKUM-WELTON, RN, PCM

Danielle earned her Bachelor of Science in Nursing from Shepherd University in 2014. For the past 10 years, she has been a dedicated member of the Grant Memorial HomeCare team, serving as both a field nurse and Patient Care Manager. Danielle enjoys caring for patients in their home environments, where they feel most comfortable. Danielle's roots are in Seneca Rocks and now calls Hardy County home. Outside of work, she enjoys life on the farm with her husband and two children. She also loves camping, fishing, hunting, and cheering on her son at his sports games.

Meet Cindy Meek-Hartman, a proud native of Grant County and Registered Nurse with Grant Memorial HomeCare in Petersburg. She earned her associate degree in nursing from Southern West Virginia Community College in 2005. Cindy has been a valued staff member of the Grant Memorial Homecare family for nearly 20 years. Known for her compassion, dedication, and wealth of experience, Cindy consistently provides exceptional care and tirelessly advocates for her patients and their families. When she is not working, she enjoys spending time with her new grandbaby, Ellie and camping with her family.

Meet Charlene Fertig, RN, Executive Director of Grant Memorial HomeCare in Petersburg, WV. Charlene graduated from West Virginia University with a bachelor’s degree in science/nursing and has been part of the Grant Memorial HomeCare family for over 30 years. Working in a variety of roles throughout her tenure, Charlene brings a wealth of experience, deep compassion, and a steadfast commitment to putting her patients’ needs first.

When she’s not working, Charlene, an East Hardy native, can be found cheering on her alma mater’s high school football team under the Friday night lights – where she also fills in as the team trainer, when needed.
