Another hot off the presses study by Hernandez-Secorun et al, this systematic review and meta-analysis found Dry Needling Therapy to be an effective treatment for Chronic Neck Pain as part of a broader plan of care in the short and intermediate terms (3 months). While all ages improved, those 40+ years old had better outcomes than those younger than 40. Another spring has sprung and that means a lot of winter-bound northern Michiganders are attacking their spring cleaning, getting their gardens ready, and getting back to their favorite hobbies or exercising. For a lot of people, the sudden increase in activity leads to aches and pains from doing too much too soon after too little for too long. A lot of people have back, buttock and leg pain. Pain in the butt with shooting pain down the thigh and into the leg-- it might be sciatica, or it could be referred pain from the gluteal muscles. Both can be treated quickly and effectively with physical therapy-- but the treatments are very different depending on if it's an irritated nerve or a knotted muscle. This study found that "Among patients with symptomatic knee osteoarthritis, intra-articular triamcinolone (steroid injection), compared with intra-articular saline, increased cartilage volume loss and had no effect on knee pain over 2 years." Steroid injections might be most appropriate for those with severe arthritis and a pending joint replacement. Otherwise, you're looking at short term relief (at best) for the price of accelerating the degenerative changes in the knee that made you seek out the injection in the first place. One of the best uses for Blood Flow Restriction Rehabilitation or BFR is for post-surgery physical therapy when a patient cannot tolerate aggressive loading of injured bone, muscles, tendons, or ligaments. This man had significant atrophy / wasting of his thigh muscles after his ACL repair. Here's how his right thigh musles "reactivated" over one month with physical therapy with BFR therapy and good protein intake (for context it would take 3-4 months with traditional exercises).

We've been using Blood Flow Restriction Therapy (BFR) since 2016 to rehabilitate strained muscles, ruptured tendons, torn ligaments, factures and age-related muscle wasting. BFR is a "bio-hack" that get injured patients back to normal 2-3x faster than they can with traditional exercise.
