POSITION SUMMARY:
Under the supervision of a registered nurse, assist in coordinating and providing nursing care for clients in home and school settings. Perform initial and on-going focused client assessments. Participate in teaching home medical management to clients and their caregivers. Work with the RN to facilitate problem resolution and achieve individualized goals and outcomes. Collaborate with other members of the health care team. This position requires travel within the Tri-State area.
RESPONSIBILITIES:
-Provides skilled nursing care to clients including, but not limited to:
A. Initial and on-going focused client assessments a directed by RN/Plan of Care
B. Client and caregiver teaching
C. Assisting in development and revision of the Plan of Treatment and Care Plan
D. Implementation of physician orders
E. Monitoring client response
F. Responding to emergency client needs
G. Psychosocial support
H. Evaluating and reporting findings to supervising RN
-Completes all required client care documentation prior to the conclusion of scheduled shift and reports accurately to parent or caregiver.
-Completes clinical documentation including assessments, interventions and patient response to interventions per One Care, Inc standards.
-Submits all required client care documentation and reports on time.
-Communicates to RN supervisor, physicians and other health care providers any changes in the client's condition and assists in resolving identified problems.
-Takes professional responsibility for self-protection during required work in potentially hazardous situations. (i.e. handling of blood & body fluids).
-Represents self and organization in a professional and ethical manner at all times.
-Complies with all applicable organization policies and procedures.
-Attends scheduled client care conferences, in-services, departmental meetings etc. as requested.
-Works effectively with client care team to clinically and financially manage the clients care.
-Maintains professional licensure, certification and health requirements as well as demonstrating clinical competence.
-Participates in the orientation of other nurses as requested.
-Performs other related duties as assigned or requested.
WORK STANDARDS:
-Maintains confidentiality of client information.
-Exhibits timeliness by arriving 5-10 minutes early to receive report from preceding caregiver.
-Completes shifts as scheduled.
-Exhibits an attitude that promotes harmony and goodwill in the workplace.
-Displays a neat, clean and professional appearance at all times.
