A 20-year-old woman presented to the ER a few times with severe abdominal pain, nausea, and vomiting coinciding with her menstrual period. She was found to have developed posterior reversible encephalopathy syndrome (PRES) and was later diagnosed with acute intermittent porphyria (AIP), a rare heme synthesis disorder that is often misdiagnosed.
