Time after time, when a woman comes in for a breast augmentation consultation and looks at dozens of before and after photos, she is relieved to find out that she isn’t the only one with breasts of different sizes.

Breasts are sisters, not twins. The difference might be slight or more obvious. And while I cannot transform sisters into twins, I can make them look like sisters from the same family.

While some women are bothered by a size difference that’s as small as a few tablespoons, others don’t notice or don’t care. Some women address their concern by adding padding – or a “chicken cutlet” – to one cup of their bra; others opt for cosmetic breast surgery.

If there’s a difference in the volume of the breasts, I can make them closer in size by enlarging them with breast implants of different sizes or by augmenting the smaller breast.

Can you go skydiving with breast implants? You might think that parachute straps could harm your implants, but they won’t. Your implants aren’t that fragile, and you’re not that fragile either. Full speed ahead!

While few of my patients go skydiving, all of them ride home after their same-day surgery. They can safely buckle up their seat belt; that won’t harm implants either. I’ve had patients who buckled up and rode home the eight-plus hours from Philadelphia to Central Ohio right after surgery. For comfort, some have placed a small square or rectangular pillow between their chest and the upper-body seat belt.

I’ve also taken care of women who are Triple Crown bodybuilders and kickboxers who go back into competition after getting breast implants. They tell me they feel comfortable getting back to their sport at around six weeks after surgery. None of their implants have deflated.

As Breast Cancer Awareness Month comes to an end, it's important to know that you can safely get a mammogram if you have breast implants.

Breast implants are designed to be sturdy. In fact, one company tests its implants by exerting nearly 55 pounds of force on them repeatedly, up to 6.5 million times. By contrast, a routine mammogram exerts about 25 pounds of force when it compresses the breast, and if a woman had 50 mammograms in her lifetime, it would be a lot.

When you schedule your mammogram, it’s important to let the facility know that you have breast implants. To maximize the amount of breast tissue that can be seen, the technician will gently displace (push up) the implants and will take extra views of each breast.

More than one-third of a million women have breast augmentation surgery each year, which means that radiologists are likely to have experience evaluating breasts with implants.
In a small percentage of cases, implants cast a shadow on breast tissue and partially obscure the view, making it more difficult for the radiologist to detect an abnormality.

When implants are placed behind the muscle, there is less shadow cast and less chance of interference.

This past summer, I was invited to speak at a plastic surgery conference in Uzbekistan. I didn't know what to expect in such an unfamiliar and exotic place. I was so pleased by my colleagues' warm welcome and hospitality and I enjoyed the sharing of our expertise with each other. It's a beautiful country full of history from the ancient Silk Road.

I’m excited to announce that a fully illustrated Kindle version of “The Scoop On Breasts: A Plastic Surgeon Busts the Myths” is now available on Amazon.

Back in 2013, I co-authored the book with my wife, Joyce. In it, we candidly answered more than 200 actual patient questions, in chapters on breast development and breast health, saline and silicone breast implants, choosing a cup size, breast augmentation surgery, the I.D.E.A.L. Breast Lift procedure that I created, and more.

We were honored to receive several awards – in the categories of women’s health, humor and design.

At that time, the paperback version had 200+ photos and illustrations, but the Kindle version was limited to just eight. Now all of the art is included. Many thanks to Anoki Casey and Cindy Hodgson, who dressed up our original manuscript with fabulous photos and illustrations - and who made us look so good! A few years ago, I was invited to contribute two chapters to an international plastic surgery manual focusing on breast augmentation, tuberous breasts, and breast lift surgery. I was also honored to be asked to write the Foreward for the volume.

I submitted the material and I've been waiting patiently.

Last week I got a holiday gift! "The Manual of Cosmetic Medicine and Surgery: Volume 2 – Breast Reshaping" was just released by Springer Publishing.

One chapter is about the technique I created in 2003 for Simultaneous Augmentation Mastopexy, a breast lift with implants, performed at the same time.

The other chapter focuses on my simpler, less-invasive technique to correct tuberous breasts using saline implants in a one-stage procedure.

I'm proud of the accomplishment and grateful for the opportunity to share my experience with a wide audience.

As a plastic surgeon whose practice is focused solely on cosmetic breast surgery, I've heard thousands of women’s thoughts about their breasts and their surgical goals. I could sum up what I’ve learned like this: Don't make assumptions!

Don’t assume that: . . many of my patients are exotic dancers.
Only about 1% percent are. The other 99% includes teachers, nurses, company executives, hairstylists, construction workers, doctors, ballroom dancers, stay-at-home moms, bartenders, fitness trainers, engineers, and women in all branches of the military.
. . women get breast implants to attract attention.
“Do you want to be proportional, turn heads or stop traffic?” That’s what one of my colleagues asks his prospective breast augmentation patients. I’ve tried it. Most women tell me they want their figure to be more proportional and they want to fit better in their clothes. Some say they wouldn’t mind turning heads now and then. Not many want to stop traffic. Ironically, some of my patients are policewomen who literally stop traffic – but they would rather not do it with their breasts.
. . women get breast implants to make their partners happy. Only two women out of thousands of women have ever told me that their partner wanted them to get implants. Instead, women have consistently told me that their partners love them exactly the way they are, and they are having surgery to make themselves look and feel better.

In 2013, my wife, Joyce, and I wrote a book together - “The Scoop On Breasts: A Plastic Surgeon Busts the Myths.” Later that year, the International Book Awards named us Winner in the category of Popular Culture! We were thrilled.

Last week, we checked out the organization's website and found a list of more recent award-winners. It turns out that we’re in really good company: A Queen. The Pope. And one of our favorite good/bad guys.

Julie Andrews – the Queen in The Princess Diaries – won for "A Memoir of My Early Years."

Pope Francis won for his inspirational book: "On Hope." By the way, long before he was the Pope, he worked briefly as a nightclub bouncer.

Bob Odenkirk, star of Breaking Bad and Better Call Saul, won for his book of humor: "A Load of Hooey."

Will people be able to tell that I got breast implants?

Most women who come in for a breast augmentation consultation tell me that want to look natural, be proportional, and fit better in their clothes. They don't want people to notice.

When breast implants are placed behind the pectoral muscle, the muscle blunts the part of the implant that peeks out of a tank top, bra or bathing suit, creating a more natural look. When breast implants are placed in front of the pectoral muscle, only skin and breast tissue camouflage the implants, and they might look unnatural, especially if a woman is very thin. Breasts might also look unnatural when the implants are too big for a woman’s frame or are out of proportion with the rest of her body.

After surgery, patients tell me that their most observant friends and family members are often aware that something is different about them, but they can’t pinpoint what it is. These curious people ask questions like, “You look great. Are you just back from vacation?” or “That’s a cute outfit. Is it new?”

Most women look natural and proportional after their surgery and can keep it a secret if they are so inclined.

I'm honored to have been named a Distinguished Fellow of the American College of Osteopathic Surgeons at the national conference in Austin, Texas in September. The DFACOS designation is conferred upon ACOS members who have demonstrated their commitment to the practice of surgery, surgical teaching, authorship of professional papers, and participation in local, state and national organizations. I'm delighted that my latest article – "The Underappreciated Saline Breast Implant" – was recently published in Aesthetic Plastic Surgery, the journal of the International Society of Aesthetic Plastic Surgery.

While both silicone gel and saline breast implants are available for cosmetic breast surgery, I've come to appreciate saline implants, both for the advantages it offers to the patient and the surgical advantages it offers to me.
