At Temple Health, we recognized a critical need in our community — individuals at high risk for stroke who need extra care and support beyond what primary care alone can provide.

That’s why we created the Frazier Personalized Stroke Prevention Program, a collaborative approach bringing together experts like Dr. Nina O'Connor, Chair of Family & Community Medicine at Temple University Health System, E. April Baidal, Family Nurse Practitioner at Temple Health, Laksisha Sturgis, Director of Community Care Management at Temple Health and Dr. Cornelius Pitts, Assistance Professor at Temple University, to help patients take control of their health and reduce their risk of stroke.

Through teamwork and personalized care, we’re helping our community to live longer, healthier lives. Watch the full video to learn more about how this program is making a difference:

If you smoke now or have in the past, you may qualify for a lung screening. Screenings help detect lung disease early, when it’s most treatable.

Not sure if you qualify? Use our Pack-Year History Calculator to learn if screening may be right for you: https://bit.ly/43ojiFF
