I'm Happy and excited to share another wonderful, amazing successful Discharge at East Falls.!!!!! No pictures just the story.
Patient admitted to AristaCare East Falls in September 2025 s/p ICH and SAH coupled with respiratory failure. At time of admission, patient required supplemental oxygen via trach collar to meet respiratory needs. He was unable to eat or drink food by mouth and received all medication and nutrition via PEG tube. Patient was totally dependent upon staff for all activities of daily living, posture correction and repositioning, and all aspects of functional mobility. Patient made tremendous strides with all rehab disciplines and respiratory therapy throughout his stay at East Falls. Respiratory therapy was able to aggressively wean patient from trach collar to room air, and patient was able to be decannulated only 30 days after admission. Speech therapy worked to improve patient cognition and swallow function – at time of discharge patient progressed from NPO all the way to being able to eat regular diet with thin liquids without risk for aspiration. Additionally, speech therapy improved upon patient cognition, memory recall, and speech, and at time of discharge patient is able to verbally make his needs known and hold conversational speech. Throughout his work with physical therapy, he improved upon strength, balance, and independence with mobility. Patient is able to perform standing tasks with minimal assistance, perform bed mobility with supervision, and is able to tolerate being out of bed in his personal customized tilt in space wheelchair. Occupational therapy facilitated improved upper extremity strength and coordination, resulting in improved independence with ALD tasks as patient is able to perform self-feeding with independence and requires minimal assist with dressing and grooming tasks. After 6 months of high intensity rehabilitation, patient is ready for discharge. We wish him all the best in his continued rehabilitation journey at home with family!!!!
