Patient Care Coordination (PCC) is a Service Coordination Entity that serves 21 counties in the Commonwealth of Pennsylvania. PCC partners with AmeriHealth Caritas CHC, Keystone First CHC, PA Health and Wellness, and UPMC HealthChoices to advocate, assess, monitor, and connect participants to the most appropriate community services that enable and maintain their health, safety, and independence in the community. The company is dedicated to providing individualized services through a participant-centered approach with an interdisciplinary team.
Role Description
This is a full-time Service Coordinator role located in Philadelphia, PA with flexibility for some remote work. The Service Coordinator will be responsible for assessing participant needs, developing care plans, monitoring and coordinating services, conducting home visits and communicating with participants, families, and providers. Home Visits may at times be in areas distant from the main office location. The Service Coordinator will also manage operations and provide excellent customer service to participants and stakeholders.
Qualifications
- Service Coordination and Operations Management skills
- 3-5 Years Experience in Social Work Practice, Care Management or related
- Excellent interpersonal and communication skills with the ability to work collaboratively with interdisciplinary teams, participants, families, and providers
- Strong customer service skills
- Knowledge or experience in healthcare coordination, case management, or related fields
- Bachelor's degree in Social Work, Nursing, Health Administration, or related field
- Licensure or Certification in a related field preferred, but not required (e.g. RN, LSW, LPC, CCM)
Relevant skills and qualifications that would be beneficial for the Service Coordinator role include:
- Experience with Medicaid-funded programs, particularly Community HealthChoices, Nursing Home Transition, and the Office of Long Term Living
- Travel Required in position
- Fluency in languages other than English preferred, but not required
- Experience with electronic health records (ELTSS, TruCare, Helios, Envolve, HCSIS)
- Strong problem-solving and critical thinking skills
