Embry Care is proud to introduce the Liberty Pathways Initiative, in partnership with Philadelphia DBHIDS.

This program is designed to help individuals with intellectual disabilities and autism build real independence through personalized, community-based experiences.

We will be selecting up to 5 participants.

Each selected individual may receive:
• A customized independence plan
• Ongoing structure and support
• Up to $3,000 in dedicated funding based on their goals

Participants will work toward goals such as:
• Employment and job readiness
• Building friendships and social connections
• Improving health and wellness
• Exploring creative interests
• Becoming more involved in their community

This is a curated experience built specifically for each person — focused on real growth, consistency, and long-term independence.
