💙 November is National Diabetes Awareness Month 💙

Did you know that more than 38 million Americans are living with diabetes — and many more don’t even realize they’re at risk? This month is about awareness, prevention, and support — not just for those managing diabetes, but for the families and caregivers walking beside them every day.

Here at Seasons Change Home Health Care & Hospice, we understand that managing chronic illnesses like diabetes can be challenging — from balancing medications and nutrition to navigating fatigue and complications. Our nurses, aides, and social workers see the strength it takes every day, and we’re here to help carry that load with compassion and care.

Small steps make a big difference:

At Seasons Change Home Health Care & Hospice, we are incredibly proud of the dedicated and compassionate professionals who care for our patients every day. Today, we are honored to highlight one of our exceptionally hardworking team members — Jennifer Rehmus, PTA 💚

Jennifer recently completed an intensive certification program through the Academy of Lymphatic Studies, showcasing her remarkable commitment to clinical excellence and professional growth. She has demonstrated outstanding competency in all aspects of Lymphedema management and gained advanced knowledge in the anatomy, physiology, and pathophysiology of the lymphatic system.

Because of her hard work and determination, Jennifer is now certified to provide Manual Lymph Drainage (MLD) and Complete Decongestive Therapy (CDT) — allowing her to deliver an even greater level of specialized, exceptional care to our patients. 🌱✨

Our physical therapy clinicians serve our clients everyday by:
