RareMed is seeking an experienced Nurse Leader to guide our Nurse Care Navigators and elevate patient-centered telehealth support. This pivotal role contributes to our mission of providing compassionate, effective care to patients navigating complex conditions. In this role, you will:
· Mentor and develop a high‑performing team of Care Navigators
· Ensure exceptional clinical quality, documentation accuracy & telehealth excellence
· Oversee operations, performance trends, KPIs, and workflow efficiency
· Collaborate with patient, infusion centers, pharmacies & manufacturer partners across the patient journey
· Lead continuous improvement initiatives that enhance patient adherence & satisfaction

What you bring:
· Active RN + BSN
· 3–5+ years clinical experience, including 2+ years in nursing leadership
· Background in telehealth, case management, specialty pharmacy or HUB services
· Strong communication, coaching & critical thinking skills
