🍼 Reflux in babies can show up in many ways — not just spit-up.

Some common signs of reflux include:

• Frequent spit-up or vomiting
• Arching the back or stiffening during or after feeds
• Fussiness, crying, or discomfort around feeding
• Pulling away from the breast or bottle
• Coughing, gagging, or hiccups during feeds
• Wet or gurgly sounds after eating
• Difficulty settling or discomfort when lying flat

For some babies, reflux is mild and improves with time.
For others, it can interfere with feeding, sleep, and overall comfort.

✨ Reflux can also be connected to feeding mechanics, oral motor challenges, tension, or coordination difficulties — not just the stomach alone.

As an occupational therapist, I love using pacifiers with babies to help with feeding and regulation. Pacifiers are a great tool to work on suck strengthening, help reflux, and calm a baby. Pacifiers can also help stimulate digestion. But pacifiers should not be used long term. The longer a baby uses a pacifier the harder it is to wean from using a pacifier. After six months of age, pacifiers use are no longer used for non-nutritive sucking and used more to meet sensory needs.

The American Academy of Pediatrics and the American Academy of Family Physicians recommends weaning children from pacifiers from 6-12 months to prevent ear infections. Pacifier use after three years of age is associated with a higher incidence of malocclusion.

Finding a replacement for the pacifier might be needed if a baby has a hard time weaning.

Feeding therapy should be fun. Children learn best through play. A good occupational therapist will make therapy look like play. What people sometimes don’t see or understand is all the things that occur before, during, and after therapy. Feeding therapy can be complex and there are many things to consider when planning a treatment session and making recommendations. There are no recipes to follow. Each child is unique and requires individual treatment planning.

Dysphagia is a medical term for swallowing difficulties. Some people with dysphagia may aspirate. Aspiration happens when food, liquid, or other material enters a person’s airway and eventually enters the lungs by accident. Aspiration may happen due to difficulty with swallow coordination, impaired sensation, structural issues, or decreased strength. Aspiration can also happen if a child has reflux. Aspiration can cause serious health issues such as pneumonia or chronic lung disease. Aspiration may be silent or present with symptoms such as:

CoughingChokingThroat Clearing
Watery Eyes
Congestion
Desaturation
Bradycardia
Color Changes
Wet Sounding Voice
Refusing to Eat

Children who have difficulty eating or drinking may prefer certain foods because it is easier to swallow. Impaired oral motor skills can lead to difficulty swallowing and increase the risk of aspiration depending on the severity. If your child has any signs or symptoms of aspiration or you have concerns about your child’s feeding, contact a feeding therapist trained in feeding and swallowing. Signs and Symptoms of Oral Dysfunction
Oral dysfunction can be present at birth or develop due to poor orofacial development. Oral dysfunction are sometimes present in children with poor head shape, high palate, oral restrictions, cleft lip or cleft palate, or a retracted mandible. Children that present with asymmetry, torticollis, plagiocephaly, hypertonia, or hypertonia may also have some type of oral dysfunction.

Signs of symptoms of oral dysfunction include:

• Mouth breathing
• Compensatory oral motor skills
• Difficulty breast feeding
• Difficulty bottle feeding
• Difficulty transitioning to solids
• Picky eating
• Difficulty controlling saliva
• Oral seeking
• Poor tolerance to tummy time
• Delayed speech
• Delayed gross motor skills
• Impaired swallow
• Sleep issues
• Oral habits

If you notice any of these signs or symptoms, consult with a pediatric feeding therapy to get help for your child. Addressing these issues earlier can prevent other problems from developing. The wait and see approach may results in these issues taking longer to resolve.
