Associates in Ob/Gyn is permanently closed. We are no longer able to provide you with copies of your medical records.

You can request copies of your records by contacting Shoreline Records Management at www.shorelinerm.com and selecting the "records request" tab on the home page. You can also reach out to Shoreline by phone 631-435-2900 or by email info@shorelinerm.com. You will need to fill out an authorization form. There is a charge for their services. I feel confident that this will be an easy and efficient process.

You may have heard that I am going to be back to work. I am currently planning to work part time seeing gyn patients with Dr. Jeff Dodge at Lake Champlain Ob/Gyn. We are still working out the details and starting date; stay tuned.

Our practice is closed now but you can still get your medical records!

We will need a signed release from you so that we can send your records to another office, or so that you can pick up a copy of your records for you to hold until you choose a new provider. There are blank forms outside our front door and a locked mailbox is there; signed forms can be left in the mailbox. We are checking phone messages and the mailbox daily, so don't be afraid to reach out to us.

As a reminder, we can no longer order tests (like mammograms) or refill prescriptions, so you will need to reach out to a new gyn or to your primary care provider.

If you have an emergency, you can to the CVPH ER if you are unable to reach another gyn physician.
