We make many of the services you would otherwise receive in a hospital available to you in the comfort of home. We assembled a team of health care professionals specifically tailored to each patient’s needs. We follow a plan of care completed by a physician that is unique to each patient, utilizing one or more of our many clinicians, caregivers and services. This could include a combination of home – based treatment, rehabilitation after an operation or injury, wound care, symptom management, counseling, or respite care.
We use cutting edge software and technology to make our work as seamless and efficient as possible, while maintaining real, person – to – person interactions between patients, their families, our staff and other providers. Qualifying for Home Health Care
To qualify for home health care services through Medicare, you must have a medical need for the skilled service or services of a qualified healthcare professional.
You must also meet Medicare’s definition of “homebound”, which, generally, means that leaving the home requires taxing effort, causing you to, therefore, leave the home infrequently, if at all. A patient may be temporarily homebound due to an illness or injury, for example.
A Plus Home Health Care, Inc. must also receive an order from your physician to provide home health care services, which will lay out your specific plan of care. The A Plus Home Health Care, Inc. team will then coordinate with your physician directly.
If qualified, Medicare pays 100% of the fees for these services.
A Plus Home Health Care, Inc. provides home health care services in assisted living facilities as well as in private residences.
