I was going through literature and I found this gem out of the Ukraine.
If we treat upper back pain w/muscle spasm utilizing Complex Therapy we can have a 90% plus success rate.

I have to share with you all an email that I just received from the NOI group. This is an organization dedicated to teach practitioners of all stripes where the current evidence on pain stands.
This email was drafted by Dr. David Butler PHD. I thank him for his work in my practice daily! I could not have voiced this concern I see and hear all the time. Enjoy!

"The folklore surrounding intervertebral disc injury has inspired some wildly descriptive and unhelpful language. Slipped disc wins by a mile, but you can also blow a disc, bulge a disc, pop a disc, even say I've got a disc. I once heard someone say that they had mulched a disc! You could use more medical terms and say you have a ruptured, herniated or pr*****ed disc. Googling images of prolapse is definitely not helpful.

I heard a new one the other day that made me recoil. "The doc' said I've shunted a disc out in my neck.” I didn't know how to respond initially! I thought shunt... that's what trains do. Shunting is a powerful and almost unstoppable force. The term is quite ideophonetic as the sound and the association provoke ideas and imagery. I thought of a shunting train moving between other trains at a railway station. And then I thought you can't really unshunt either. What a mean word when applied to an already linguistically unstable body part.

I couldn't let it go of course, but I couldn't resist starting my story telling with "let me stop you in your tracks for a moment. Your neck is no railway station and it's certainly no train crash in there either".

I am sure it's best to leave shunted for the railway yard.

It is a beautiful and accurate statement on the current knowledge base. My mind works like he talks, fast and furious. Try to keep up, because I could not have described these complex concepts better than he did.

This quote of quotes (if that is a thing) is taken from the NOI group. Their mission is to coach health care providers to consider the ramifications of their language based on current pain research. The below sample spoke to me and I hope it does the same to you!
"A story might go something like this. “Wear and tear – that’s a term used for machines. Health professionals have used it to describe x-ray changes, but it’s not a good descriptor. There is rarely any tear, more likely just a bit of wear which is quite normal and tells us how active you have been. Actually saying or thinking wear and tear can keep you on alert and more sensitive – maybe saying ‘a bit of normal wear’ rather than ‘wear and tear’ is more accurate.” You could also add “when our bodies wear, they always adapt and there will always be some repair”. So ‘wear and repair’ or ‘when it wears, it also repairs’ could be healthy alternatives."

I found this little gem that I found very interesting and congruent to my clinical experience. Some Problems with the study are as follows: small sample size, only women, and Manual Therapy techniques are poorly described, BUT “Conclusion: Manual therapy and surgery had similar effectiveness for improving self-reported function, symptom severity, and pinch-tip grip force on the symptomatic hand in women with CTS. Neither manual therapy nor surgery resulted in changes in cervical range of motion.”

In manual therapy/chiropractic we look at the entire pathway of the nerves to see what conservative care options have the potential to improve your quality of life for a whole host of ailments. I feel this is prudent to exhaust conservative treatment prior to attempting the most invasive treatment (surgery) first as long as there is no red flags. This is just one more example of our potential here at Structure and Function! Thanks!
