(Thai massage uses 4 main client positions for full-body work)

1. **Supine (Lying on Back)**
Legs/feet pressed and stretched, hip openers, gentle twists, chest/shoulder releases. Great for starting the session and working energy lines.

2. **Side-Lying**
Side stretches, hip/shoulder mobilizations, arm/leg compressions. Ideal for addressing one side at a time, especially back & hip tension.

3. **Prone (Lying on Stomach)**
Back, glutes, hamstrings, and calf work with palm/foot pressure and deep stretches. Excellent for relieving lower back pain and opening the front body.

4. **Seated**

In my experience, almost every massage I have received whether spa, clinical, or therapeutic begins face down, working on the back first, because that is where the client feels pain.

But pain location does not always mean problem location.

In traditional Thai Massage, the Sen lines originate around the abdominal and anterior body, then connect and influence the posterior structures. Modern anatomy actually supports this idea.

Human movement is mostly performed in flexion:
• sitting
• driving
• phone and computer use
• forward-focused activities

Add gravity to this pattern, and over time the anterior chain becomes shortened and dominant:
• hip flexors tighten
• pectorals shorten
• abdominals become overactive
• breathing shifts forward and superior

When the front tightens, the back must constantly work to keep us upright. The posterior muscles become overloaded stabilizers, not necessarily short muscles.

This explains why many people have back pain even though the real driver is anterior restriction.

When treatment starts face down and focuses only on the painful area:
• we often work repeatedly on already fatigued posterior muscles,
• sometimes lengthening tissues that are already inhibited,
• which can leave clients feeling sore or worse after the session.

Clinically, this is not because back work is wrong, it is because the sequence matters.

If we first address the anterior body, breathing, chest, hip flexors, diaphragm, and abdominal tension the posterior muscles no longer need to guard.

Then back treatment becomes more effective and requires less force.

Interestingly, Thai Massage understood this relationship long before modern biomechanics described the anterior–posterior balance and kinetic chain concepts.

From my clinical experience:
when you change the front of the body, the back often changes by itself.

Treat the cause, not only the complaint.
