Balloon-assisted coiling.

CTA with 3D reconstruction and yellow arrow shows the aneurysm popping out near the Sella turcica.

3D angiogram shows the aneurysm nicely.

Left fronto-temporal craniotomy with subfrontal approach for resection of metastatic tumor. Preservation of Brocas area (preserving speech).
Before tumor resection the lateral ventricle was compressed with mass effect from the tumor pushing the brain to the right.

Late 50s patient, prior ruptured brain aneurysm clipped >25 years ago. Did not want to get another open craniotomy. No good choices due to wide necked aneurysm with ICA terminus diameter approx 8mm for Stent-assisted, flow diverter and even balloon-assisted coiling not good options since there are sizes available.
Neurosurgeon needs to keep up with technology and know what is out there for brain aneurysm treatment.
