To all of our current patients, we have become aware that another company is infringing on our logo and adding the word mobile to trick our current patients into signing another consent form.

Please be aware this is not 5 Star Medical Care LLC and we are taking action. 5 Star Medical Care LLC is up and running and still seeing patients as usual. If you have been asked to sign another consent form with the word "MOBILE" in it please contact us at the number or email below ASAP, as we are in the process of serving this false company infringing on our logo, name and misleading and fraud of redirecting our patients.
