While we often think of trauma as a single "major" event, it is more accurately defined by how your nervous system processes overwhelming stress. It occurs when an experience exceeds your ability to cope, leaving a lasting imprint on your brain and body. Understanding trauma starts with shifting the question from "What is wrong with me?" to "What happened to my internal sense of safety?"

How does your definition of trauma change when you view it as a physical response rather than just a memory?

Save this post as a reminder that your reactions are valid responses to invalid circumstances 🌸

Scaling a mental health practice is a journey of constant evolution. 📈

Dr. Brad Larsen, PsyD, recently sat down with Joe Sanok to share the "conversational gems" gathered from his experience building and managing Portland Mental Health and Wellness.

With over 130 clinicians on the team, the challenge is balancing operational excellence with the human-centric nature of therapy. Brad discusses:
→ How Nichiren Buddhism informs his executive decision-making.
→ Applying Gestalt principles to organizational structure.
→ The "vision-bearing" roles within a large-scale practice.

A huge thank you to Joe and the team at Practice of the Practice for the invitation!

Every leader hits a wall. In his latest conversation with Joe Sanok on the Practice of the Practice podcast, Dr. Brad Larsen gets honest about the weight of leadership.

Managing 130+ clinicians isn't just about logistics; it’s about building a culture where the vision is shared so deeply that the community carries it forward, even on the hard days. Brad dives into how the principles of Gestalt and Buddhism help create a resilient organization that supports both the providers and the patients.

How do you lead a team of 130+ clinicians while staying grounded in your values? 🧘‍♂️

Our Founder and Executive Director, Dr. Brad Larsen, recently joined Joe Sanok on the podcast to discuss a unique intersection: Leadership, Nichiren Buddhism, and Gestalt therapy.

Scaling a practice to this size requires more than just business strategy—it requires a deep commitment to presence, human connection, and a clear spiritual "why." Whether you’re a solo practitioner or leading a large organization, Brad’s insights on maintaining clinical integrity during growth are a must-listen.

📢 Now Hiring: Postdoctoral Psychologist Residents (2026-2027)

We are seeking clinicians and lifelong learners for our Postdoctoral Residency. This is a unique opportunity to bridge the gap between training and licensure within a community-focused practice that prioritizes relational, process-oriented, and liberation-focused mental health care.

We are looking for PsyD or PhD candidates who are passionate about deep clinical work and committed to equity and humility. Residents will choose between two specialized tracks: our Intensive Outpatient Programs (including specialized LGBTQ+ care) or our Outpatient Psychotherapy and Assessment rotation.

At PMHW, we value the lived experiences of people of color, LGBTQ+ individuals, women, people from working-class backgrounds, and others from marginalized communities, and we strongly encourage candidates with these identities to apply.

What is an IOP program? An Intensive Outpatient Program offers structured, supportive mental health care for individuals experiencing moderate to severe symptoms that impact daily life.

Our IOP provides at least 10 hours per week of therapeutic support, including group and individual therapy, along with psychiatry when needed. Group sessions create space to build coping skills, strengthen self-awareness, and connect with others who understand what you are going through.

If you are looking for meaningful support while continuing to live at home and maintain daily routines, IOP may be a helpful next step. Learn more through the link in our bio.
