Housecall Providers Over those years, we have served over 15,000 patients.

Housecall Providers is a Portland-area nonprofit that delivers personalized, proactive primary and palliative medical care and hospice services to seriously ill and homebound adults who live throughout the Portland metro area. For over 28 years, Housecall Providers has been a leader in demonstrating the quality, medical efficacy and cost-efficiency of providing primary medical care in the home to medically complex and disabled homebound patients. Over 95% of our patients have been satisfied or highly satisfied with Housecall Providers’ services according to surveys. Our current programs include team-based primary care (PCP, nurse, social worker and spiritual care) Advanced Illness Care (community-based palliative care) and hospice.

Al, a beloved public defender and later an administrative law judge in Multnomah County, lived with post-polio syndrome for years before a sudden decline late in life. When he began to weaken significantly his doctor referred him to hospice, though not with the provider that would ultimately make the most meaningful difference for his wife Helen.

Morning well spent at The Community for Positive Aging writing holiday cards for their members and volunteers. πŸ’Œ

is a nonprofit, age-positive hub with a mission to create a healthier, more inclusive, and deeply connected Portland where adults 55 and older thrive through innovative programming, compassionate support services, and person-centered care.

Improving lives by bringing health care home

For 25 years, Housecall Providers has been a leader in demonstrating the quality, medical efficacy and cost-efficiency of providing primary medical care in the home to medically complex, disabled, and homebound patients. Throughout those years, we have made over 180,000 house calls and served over 12,000 patients. Last year we admitted over 550 new patients and made nearly 10,00 house calls. Over 96 percent of primary care survey respondents said that they would recommend our services to their family and friends. Additionally, 92 percent of hospice survey respondents gave an overall positive rating of care. Housecall Providers is one of the sites involved in the national Medicare demonstration project, Independence at Home – and the only one west of Texas. In May of 2017, Housecall Providers became a member of the CareOregon family.
