Housecall Providers Over those years, we have served over 15,000 patients.

Housecall Providers is a Portland-area nonprofit that delivers personalized, proactive primary and palliative medical care and hospice services to seriously ill and homebound adults who live throughout the Portland metro area. For over 28 years, Housecall Providers has been a leader in demonstrating the quality, medical efficacy and cost-efficiency of providing primary medical care in the home to medically complex and disabled homebound patients. Over 95% of our patients have been satisfied or highly satisfied with Housecall Providers’ services according to surveys. Our current programs include team-based primary care (PCP, nurse, social worker and spiritual care) Advanced Illness Care (community-based palliative care) and hospice.

This November is Lung Cancer Awareness Month. We’re taking a moment to recognize the people and families living with lung cancer and the strength they show every day.

NPHI members across the country care for patients with all types of lung diseases, including lung cancer. They continue to share stories, resources, and tools to help patients get the support they need.

We also offer an Advanced Cancer Care Patient & Caregiver Guide, created with the American Cancer Society. It’s an easy-to-use resource to help families understand their options and feel more prepared.

November is both National as well as . Today we were reminded of the more than 192,000 Oregonians providing unpaid care for a person living with Alzheimer's or other dementia at the Alzheimer's Association Caregiver Conference.

Also known as the McGinty Conference on Alzheimer's, Aging & Brain Health, this conference is designed for or , and has provided quality, person-centered education and training to the community for 27 years.

Housecall Providers Hospice Liaison Laura Brooks and CareOregon Vice President of Health Plan Operations Walter Burkhartsmeier were happy to be in attendance. Laura is an Alzheimer's Association Community Educator in training and Walter serves on their leadership board of directors.

Thank you to the Alzheimer's Association - Oregon & Southwest Washington Chapter, today's keynote speakers, and everyone involved for such an insightful and empowering conference!

Housecall Providers Hospice Liaisons Laura Brooks and Heather Young had a great time volunteering at the recent Oregon Health Forum event, which CareOregon is an annual sponsor of.

The provides a public venue for healthcare leaders, consumers, policymakers, and advocates to collectively and constructively address timely health policy issues. The topic of last week's event was "Beyond Federal Dollars: Securing a Healthier Life for Oregonians."

We discussed how Oregonians can best direct resources to address health inequities with maximum impact in the wake of federal cuts, and what state officials, funders, nonprofits and those in the private sector are planning now.

Improving lives by bringing health care home

For 25 years, Housecall Providers has been a leader in demonstrating the quality, medical efficacy and cost-efficiency of providing primary medical care in the home to medically complex, disabled, and homebound patients. Throughout those years, we have made over 180,000 house calls and served over 12,000 patients. Last year we admitted over 550 new patients and made nearly 10,00 house calls. Over 96 percent of primary care survey respondents said that they would recommend our services to their family and friends. Additionally, 92 percent of hospice survey respondents gave an overall positive rating of care. Housecall Providers is one of the sites involved in the national Medicare demonstration project, Independence at Home – and the only one west of Texas. In May of 2017, Housecall Providers became a member of the CareOregon family.
