If you have had a heart attack or stroke or a diagnosis such as peripheral artery disease or diabetes, PLEASE make sure you are getting optimal lipid lowering therapy. Numerous major studies have shown that if you lower your LDL-c or nonHDL-c to certain levels you can avoid another major adverse cardiac event. If you are on a medication called a statin, ask your doctor if it is high intensity. If you had symptoms while on a statin, you can try another statin at a lower dose or take the statin drug every other day or even once a week. In cases where you need to lower your cholesterol even further despite being on a statin medication, ask your doctor if there is anything else they can do. The next steps in treatment include medications called ezetimibe and PCSK9 inhibitors. Happy Thanksgiving to everyone this week. Before I dig into the benefits of cranberries, there are two very important aspects of Thanksgiving that should be emphasized. Showing gratitude and spending time with friends and family. These are powerful methods of boosting health and longevity. Yet, most people don't even think of these things when they worry about their health. Don't wait for the holidays to connect with friends and family and make it a daily ritual to express gratitude. I promise you will reap tremendous benefits for years to come.

As for cranberries, they can be an incredible source of anti-oxidants. Anti-oxidants help reduce inflammation in the body, improver the health of your arteries and boost mood, just to name a few benefits. The number one category for antioxidant rich foods are herbs and spices. Second are berries. Out of all the commonly eaten berries blackberries top the list. But close to the top are cranberries. Almost 2000% more than bananas, which have almost none.

As for all foods, you want to focus on minimally processed sources of cranberries. So a cranberry stuffed pie, cranberry sauce and even cranberry juice are not ideal. Whole fruit (fresh or frozen) or dried cranberries should be your target sources.

So this thanksgiving get an extra serving of cranberry sauce in and your body will thank you!

When you visit your primary care doctor for an annual check up a standard lipid panel is ordered as part of routine labs. This lipid panel includes what doctors describe as the 'bad' LDL cholesterol and the 'good' HDL cholesterol. In truth, that is an over simplification but it gets the main point across to our patients. It is always nice to step back and look at the big picture. Why do we even care about your LDL cholesterol? It is to make an educated guess on your future risk of getting a heart attack or stroke or diabetes, etc. So when your doctor says your cholesterol levels are normal, you should naturally feel relieved. But I urge to dig deep into the numbers and truly understand your risk. This is important because if you realize your risk is elevated early in your life you can make the necessary lifestyle habit changes and get back on track. we spend time with our patients and educate them on the markers that best stratify your heart disease risk such as non-HDL, apolipoprotein B, LDL particle number and size, HDL size, hs-CRP and lipoprotein (a) level.

The normal LDL cholesterol level most doctors recommend is different based on your overall risk level and is either less than 160 or 130 or 100 mg/dl. The level of LDL cholesterol we recommend for ALL patients is between 50-70 mg/dl. At that range the chances of any atherosclerosis (plaque build up in your arteries) progressing is minimal.

This was based on analysis from 3 large studies conducted by Harvard researchers that looked at over 200,000 US adults. I applaud those that have chosen to consume a plant focused eating pattern as it will not only help your health and wellbeing but improve the economic and environmental burden to society as a whole. But a plant based diet can technically be beer and chips! Focus on whole foods such as whole vegetables, whole fruits, whole grains, legumes, nuts and seeds. This is what will help you reach your goals of improving mood, losing weight, controlling blood sugars, improving cognition, etc. In the same study mentioned above, a healthy plant based diet reduced risk of diabetes by 34%.

Around 30 million people in the US have diabetes and another 80 million have prediabetes. Why should we care? Because diabetes leads to heart attacks, strokes, blindness, kidney failure and leg amputations in the people we love. If that wasn't enough the economic costs to the individual and to society are huge. In 2017, $327 billion was spent in direct and indirect costs.

I sometimes think about how many suicides could have been prevented if we as primary care providers simply helped the patient sleep better. It's just one aspect of a multifactorial disease but good quality and quantity of sleep is crucial to the success of the management of mental health. There are two aspects to helping people sleep better. First is the education and knowledge of how to sleep better. Secondly, the actual implementation of that knowledge requires motivation, accountability and behavioural therapy strategies. our program for Insomnia and for Depression/Anxiety helps with both aspects of treatment.
