Web: www.hrchc.org | Phone: 757-393-6363 “WE CARE SO YOU CAN BE HEALTHY” Hampton Roads Community Health Center (HRCHC), formerly known as Portsmouth Community Health Center, Inc., opened its doors as a federally qualified health center on May 14, 1995, in Portsmouth, Virginia. In 2001, Portsmouth Community Health Center, Inc. expanded its services to Norfolk, VA, with the launch of its Park Place Family Medical Center, located on the corner of 35th and Granby Streets, directly across from Lafayette Park and the Virginia Zoo. Our mission, “We care so you can be healthy,” speaks to our pride in being a dynamic organization, providing high-quality, comprehensive healthcare and always being responsive to the health and education needs of the medically under-served residents throughout Hampton Roads’ communities. This is accomplished through emphasis on continuity of all patient care, as well as, case management and patient education. We offer a broad range of services to a large and varied patient population and have achieved community-wide acceptance.
