⭐ This Spotlight Sunday, as National Public Health Week comes to a close, we’re proud to recognize our dedicated CHDC Community Health Workers (CHWs), Bryanna and Gladys (pictured L-R).
"At CHDC, our community health workers effectively engage patients facing homelessness meeting them out in the community utilizing compassionate, patient centered approaches. They build trust, assess needs and facilitate appointments for patients to both internal CHDC services as well as coordinating services to external specialists. Their ability to advocate for patients and provide transportation helps to ensure continuity of care and improved access to health services and patient outcomes. Great Job Gladys and Bryanna. As part of the Montgomery County Street Medicine Program, our CHWs provide essential healthcare services to individuals experiencing homelessness in Pottstown and beyond. Founded in January 2021 by Tower Health Medical Group and Access Services, Inc., this program has served more than 800 unique individuals experiencing homelessness, with over 3,500 patient encounters to date. We are proud of the trusted partnerships that make this work possible, and look forward to continued growth, meaningful impact, and watching our team make a difference in our community every day!
