Our mission is to improve the well being of everyone by providing accessible, quality health care for the whole community.

Our integrated care model has been recognized throughout the state and combines medical and behavioral health care under one roof. Heritage Health Center is a primary care clinic. With a highly-experienced medical staff including a physician, a physician’s assistant, Licensed Clinical Social Workers, Licensed Practical Counselor and nursing staff. The clinic offers traditional healthcare services such as routine patient visits and preventative care, chronic disease management, and lab work, as well as, behavioral health services. Heritage Health Center Services are available for all people regardless of insurance status. As a federally-qualified community health center, Heritage Health Center clinic offers a sliding fee schedule for services based on household income and size. In addition, the clinic accepts payment from Medicare, Medicaid, CHIP, and private health insurance.

In honor of National Mentoring Month, we are thrilled to introduce our brand-new Advanced Practice Provider (APP) Transition to Practice Program! While this pilot program is still developing, it is now available at all One Health sites to support our staff new to primary care. This 12-month structured program is specifically designed to bridge the gap between education and practice. By providing tailored mentorship and specialized education, we are empowering our newest providers to serve our rural and underserved communities with confidence.

Comprehensive Learning: Features didactic modules, online learning, and MAVEN consults.

January is Cervical Cancer Awareness Month! It's crucial to understand that cervical cancer often doesn’t present any signs or symptoms in its early stages, making regular screening essential for early detection.

For women aged 21 to 29, it’s recommended to have a Pap test alone every 3 years. This simple procedure can help identify any abnormal cells before they turn into cancer. For women between the ages of 30 and 65, co-testing with both a Pap test and an HPV test every 5 years offers an even more comprehensive approach to cervical health.
