✅ Doctors (MDs, DOs), Nurse Practitioners (NP, PNP, ACNP, APRN, FNP) and Physician Assistant (PAs)
✅ Practicing in rural/HPSA-designated counties
✅ Provide ≥20 hours/week of direct patient care
✅ Hold prescribing privileges
