LifeStreams, LLC Who We Are...
Known as leaders in the field of rehabilitative therapies and other related services.

The entire idea behind our company is to allow you to live your life...your way.

* We have all worked in various areas of social services for over 30 years, and in that time have been involved with several states and several companies. But, we always thought, “we can do this better!” We found that many companies and agencies make things too difficult, are not well-trained in their field, and lose sight of putting the person first.

* We are Cognitive/Behavioral Therapists, Social Workers, Licensed Professional Counselors certified in brain injury and also clinical addiction, and seasoned field staff.

* We focus on a wholistic approach. We believe that a person is more that just the sum of their parts, and likewise a peron’s life is more than just one situation or time period of their life. To live a happy fulfilling life, we all need to seek a healthy lifestyle…physically, mentally, emotionally, and spiritually...whatever each of those mean to you. Only then will we truly reach our personal goals, and maintain our independence and lessen dependence on supports later in life.

* In fact, that is the inspiration for our company’s name

You opened a room where hope could breathe
and survivors gathered from every direction
carrying stories cracked by impact
yet searching for a place where strength could rise again

Mark stepped in with storms behind his eyes
a rollover survivor learning life in fragments
and the room leaned toward him with quiet understanding
lifting a weight he thought he carried alone

Elena arrived with fear woven through her voice
black ice had stolen the world she remembered
yet when she spoke the circle answered her gently
and she felt courage forming new roots in the soft light

Jordan came home from distant service
with injuries no scan could fully measure
but here his words found solid ground
and he no longer felt invisible in his own struggle

Maya moved with a changed rhythm
a medical crisis reshaped her speech and her steps
but she heard someone whisper me too
and in that moment she felt whole again

Four stories rising like lanterns in the dark
each diffe
rent yet bound together
because you made space where healing could stand tall
and let hope take its rightful place among us

Thank you for giving us a room where healing could finally speak. Thank you for seeing survivors who felt unseen. Thank you for building a circle where fear softened and hope grew. Thank you for giving Mark a place to breathe again and Elena a place to stand again and Jordan a place to be understood and Maya a place to feel whole. Thank you for showing up when so many of us were struggling to show up at all. Thank you for the light you brought into a day that felt impossible. Thank you for creating a space that turned survival into strength and strength into community. Thank you for giving me the saving burst of energy I needed to remember that I am still here and still rising.

The NASHIA Partnership Award celebrates organizations and individuals whose collaboration and expertise strengthen systems of care and promote best practices, helping people with brain injury live and work in their communities as independently as possible. NASHIA is honored to present the 2025 Partnership Award to JD Kemp, Owner and CEO of LifeStreams, LLC, in recognition of his deep collaboration, shared vision, and unwavering support for NASHIA’s mission.

With 39 years of commitment to individuals with disabilities, including 15 years focused on brain injury rehabilitation, JD has worked tirelessly to ensure that services are person-centered and in the community. As the founder and CEO of LifeStreams, JD leads with a philosophy rooted in recognizing that every person’s journey is unique. Under his leadership, LifeStreams has become known for its comprehensive model that supports community integration. JD is also a strong advocate at both the state and national levels. He serves on both the Brain Injury Advisory Board in Kansas and the Brain Injury Advisory Board in Missouri, and is also on the Board of Directors for the Brain Injury Association of Missouri. He has also supported a coalition through the ACL's Grassroots Project to help people with brain injury to advocate for themselves.

Through his decades of leadership, advocacy, and generosity, JD and LifeStreams have been steadfast partners to NASHIA. He actively serves on two of NASHIA’s Committees and annually supports the State of the States conference. Most recently, JD and LifeStreams have collaborated on a project with NASHIA, funded through the Department of Justice, to improve outcomes for individuals with brain injury in the criminal legal system. JD consistently offers insight, support and good humor to NASHIA’s efforts whenever asked and beyond.

JD’s influence and collaboration continue to shape how brain injury services are delivered. Through his leadership and partnership, JD Kemp and LifeStreams truly embody what it means to help individuals with brain injury live their lives, their way. NASHIA is honored to present JD with this NASHIA Partnership Award.

🌟 LifeStreams at the Brain Injury Association of Missouri Conference! 🌟

The LifeStreams team was proud to attend the Brain Injury Association of Missouri’s Annual Professional Conference over the last couple of days! It was an incredible opportunity to connect with professionals, advocates, and survivors who share our passion for improving the lives of individuals impacted by brain injury.

A special highlight of the event was JD Kemp, CEO of LifeStreams, presenting “Caregiver and Provider Burnout – How to Recognize, Access Resources, and Implement Wellness Strategies.” His session provided valuable insights on identifying burnout, accessing support, and promoting wellness across care systems.

We’re grateful for the meaningful conversations, new insights, and inspiring sessions that continue to strengthen our work across Missouri and Kansas.
