The Community Health Needs Assessment survey happens once every three years. The goal? Help local health leaders understand the health priorities that matter most to our community.

The online survey takes just a few minutes to complete, and your input helps guide programs and initiatives for years to come.

Plus, this year, we’re also hosting focus groups in Prince Frederick and Solomons on April 16, where you’ll have the opportunity to share your thoughts, experiences and ideas in person.
