Mental health should not be the exception to medical standards.
In every other field of medicine, when a first-line treatment fails, the standard response is escalation. If antibiotics don't work for an infection, we don't just prescribe more of the same antibiotics. We change the intervention.
Yet, in mental health, we often cycle through the same medications and therapies for months—even years—despite minimal progress. We do this not because it’s effective, but because we often forget there is a next step.
Treatment-resistant depression isn’t a “more therapy” problem. It’s an intervention problem.
At Revive Therapeutic Services, we believe in the power of Interventional Psychiatry. Technologies like TMS (Transcranial Magnetic Stimulation) exist to provide that rapid escalation when the clinical picture isn't moving.
To our fellow clinicians in Rhode Island: You don't need to deliver these interventions yourselves. But having a reliable referral pathway for when first-line care stalls is crucial for your patients' recovery.
Don't wait for a miracle. Escalate when the data says it's time.
