Community Health Centers are delivering evidence-based heart care at scale, improving outcomes for millions of patients at risk for cardiovascular disease.

📊 In 2024:

✅ 6.17 million Community Health Center patients received care for hypertension
✅ 67% of those patients had their blood pressure under control

Community Health Centers deliver proven preventive and follow-up care for patients at highest risk:

✅ 78% of high-risk patients were prescribed or taking statin therapy
✅ 74% of adult Community Health Center patients with heart disease, a history of heart attack, or prior cardiac procedures were receiving recommended antiplatelet therapy.

Cervical cancer claims the lives of more than 4,000 women in the U.S. each year, yet it is highly preventable with timely care.

Don't let the cold stop your fitness routine! ❄️ Getting 150 minutes of physical activity each week is key to boosting your mental health and decreasing risks for heart disease, type 2 diabetes, and some cancers.

Gear Up: Wear layers (coats, hats, gloves) and proper footwear to stay safe and warm outside.

Stay Inside: If the weather is too harsh, try indoor options like squats, push-ups, online exercise classes, or a mall walk!

Set Goals with F**T:
Frequency: How often will you move each week?
Intensity: How hard will you work? Aim for moderate intensity!
Time: How long will each session be?
Type: Choose activities you enjoy! Mix aerobic and muscle-strengthening exercises.
