I study the physiology of weight regulation in midlife women. I've listened to thousands of them describe the same pattern. Someone asked on one of my posts about what it means to be boarded in obesity medicine.

Here’s the tea 🫖

Only physicians license in the United States with an unrestricted license to practice medicine are eligible for the obesity board exam.

There are two pathways:
1. An obesity medicine residency or fellowship after/during family practice or internal medicine residency.

2. Physicians board certified in another specialty (like me-boarded in general surgery) who spend a minimum of 60 hours learning obesity medicine through Continuing Medical Education.

Completing either pathway makes a physician eligible to take the obesity board examination.

It’s given annually in October.

In addition to the CME, I studied for several months prior to taking the test.

I’m not lying when I say it’s one of the most difficult medical examinations I’ve ever taken. And doctors take a LOT of tests.

This is what it means to be board certified in obesity medicine.

Estrogen directs fat storage. Testosterone drives muscle. When both decline, body composition shifts independent of calories. The Finkelstein study proved estrogen deficiency drives fat gain even in men.

A man in my comments said women have zero testosterone. Women produce testosterone every day. The ovaries keep making it for up to a decade after menopause.

And men? Estradiol is the primary regulator of your bone metabolism. Every human body runs on both hormones.

HRT and GLP-1s together. Early data suggest they work better combined. The physiology supports it. And maybe we’d know more if more than 10.4% of the NIH budget went to women’s health.

It’s just physics.

Except your body drops its metabolic rate, lowers leptin by 44%, drops T3 by 39%, and defends its energy reserves with every hormonal tool it has. That’s biology determining the burn rate.

Add in the metabolic zone of chaos known as perimenopause, and there’s a predictable pattern of weight gain. In fact, I have a whole training on this on my website. Link in bio.
