Nurse Case Manager (LPN or RN) – Per Diem / PRN
Location: The Villages, Florida
Employment Type: 1099 Independent Contractor
Schedule: Flexible / As-Needed (Ideal for Retired Professionals)
Are you a retired or semi-retired nurse living in or near The Villages who still has a passion for patient advocacy and wants to utilize your skills helping others navigate the healthcare system?
We are seeking a compassionate, "people-person" LPN or RN to join our team as an As-Needed Case Manager. This is a 1099 independent contractor role designed for someone who thrives on variety, loves building local connections, and possesses the clinical "gut instinct" that only comes with experience.
The Role
In this position, you won't just be managing charts; you’ll be the bridge between patients, families, and healthcare providers. Because the needs of our community are unpredictable, we are looking for someone who can pivot quickly and handle last-minute requests with a smile. This is mainly for urgent doctor appts or home visits.
Key Responsibilities:
* Patient Advocacy: Coordinate care plans and ensure patients are navigating the healthcare system effectively.
* Networking: Build and maintain friendly professional relationships with local clinics, specialists, and facilities within The Villages. Attending local networking events 1-2 times per month.
*Communication: Provide clear, kind, and professional updates to families and interdisciplinary teams.

Requirements
* Active Florida RN or LPN License.
* Reliable transportation within The Villages area.
* Proven experience in case management, home health, or a similar advocacy-based role.
* Ability to work as a 1099 Independent Contractor.
