Our Story

Hudson Headwaters Health Network is a nonprofit system of 19 community health centers providing primary care to more than 5,600 square miles of the Adirondack North Country and Glens Falls region. The network serves all of Warren County and parts of Clinton, Essex, Hamilton, Saratoga and Washington counties.

Hudson Headwaters is the sole medical provider in much of this rural, medically underserved region. In the Glens Falls area, Hudson Headwaters is the only primary care “safety-net” provider. Our patients are served in Hudson Headwaters’ local health centers, in hospitals, nursing homes, work places, schools and in their homes.

When people in our region need health care, they look to Hudson Headwaters first. We promote healthy lifestyles, diagnose and manage illnesses, and coordinate care, while serving patients of all ages. Our medical staff will see you at one of our 17 health centers, in the hospital, a nursing facility, and even in your own home.

Our medical professionals are trained to listen so they can understand your needs and find the best way to help you and your family. With a broad selection of services – from women’s care and pediatrics, to behavioral health and imaging — we can treat most conditions effectively, and will coordinate your care for the rest.
