Transitions LifeCare - NC

Transitions LifeCare - NC Founded as Hospice of Wake County in 1979. Serves 8 counties including Wake. Oldest, largest and most experienced end-of-life organization in the area. Why?

"Hospice of Wake County, Inc. Becomes Transitions LifeCare April 30, 2014"

For several years Hospice of Wake County has offered more than “hospice” care and has served patients well beyond “Wake County.” Though we changed our name to Transitions LifeCare, our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay. The core brand name “Hospice of Wake County” no longer accurately reflected all we do. We’re not just hospice. And we serve more than Wake County. Our service territory extends well beyond Wake County so we help more patients and families in Durham, Johnston, Franklin, Harnett, Wilson, and Nash counties, eastern Chatham County and in the communities of Chapel Hill and Carrboro in Orange County. Rest assured our expertise, compassion and commitment to helping families facing a serious or terminal illness remain the same. We are still a non-profit organization with a local board of directors. We have not been bought by a large, for-profit corporation. Going forward, Transitions LifeCare is our single brand name and our service lines have corresponding names: Transitions HospiceCare, Transitions Kids, Transitions PalliativeCare, and Transitions GriefCare.

Being named Raleigh’s Best Hospice is an incredible honor. The feature highlighted what guides our work every day: “hospice is not about giving up — it’s about living fully and comfortably,” and ensuring that “individuals facing serious illness receive care that honors dignity, comfort, and choice.”

There’s something special about watching our Transitions Kids patients and families light up at our annual Fall Fest. 🍂✨ With help from over 20 dedicated volunteers and our TK teams and their families, the day was filled with laughter, games, and moments our kids will remember.

We’re grateful to McLean Farms Nursery & Ice Cream Shop for donating pumpkins and to Sunrise Community Farm Center for bringing therapy bunnies and a horseback-riding raffle — your generosity helped make the day brighter for our kids and families. 💜🧡

We had a wonderful time connecting with the Wilson community at this year’s Whirligig Festival! 💜

This well-attended event gave us a meaningful opportunity to share Transitions LifeCare’s mission and make an impact—helping more families learn about the compassionate care and support available across eastern North Carolina. We’re so grateful to everyone who stopped by our booth and helped us spread the word about the importance of compassionate end-of-life care.

Transitions LifeCare’s Community Engagement team recently visited The Stratford Assisted Living in Chapel Hill to lead an educational discussion with their leadership team.

Our Associate Director of Education, Dwan Kelsey (pictured right), facilitated a meaningful conversation on recognizing the differences between confusion, delirium, and dementia. Also pictured is Tawana Coleman, Clinical Manager at The Stratford.

We’re grateful for the opportunity to support and empower our senior care partners! 💙✨💭

We had a wonderful time connecting with local seniors at the Angier Senior Health Fair! 💜

A sincere thank you to the Angier Senior Citizens Club for organizing such a meaningful community event, and to the Angier Black River Fire Department for generously opening their space.

We always appreciate the opportunity to support older adults, answer questions, and share helpful resources with our community. 💬👏💟
