This fall, visitors to our Hospice Home were invited to choose a handcrafted heart—each one a symbol of comfort, connection, and care. 💌
As a little Feel Good Friday reflection, we’re looking back on these meaningful moments. We’re deeply grateful to the ANG, The American Needlepoint Guild, Inc and the Embroiderers' Guild of America for this beautiful donation and the warmth it brought to families at Transitions. 🫶💜

If you’re out grabbing your milk and bread, please consider picking up a new blanket and dropping it off for our Blanket Drive by tomorrow at 5 PM. We’ll deliver it directly to a patient or community member who needs it.

We are grateful to everyone who joined us, as each light represented a life, a story, and a love that continues on. Hospice care means caring for the whole family — including the furry ones. 🐾 This sweet trio was part of a family we served, and this week we were grateful to partner with Longleaf Animal Rescue to ensure they were transferred to rescue, where they’ll receive lifelong medical care and the chance to find safe, loving forever homes. No one should have to worry about what will happen to their pets at the end of life. We’re honored to help carry that burden, so families can focus on what matters most: time, love, and comfort. 💜

On January 13 & 14, our campus will once again glow with hundreds of luminaries as we honor the lives of those who have died and hold space for all who grieve. Lights of Remembrance offers a peaceful, self-guided walk through illuminated pathways, inviting reflection, connection, and comfort in community. Join us! 💜
