Transitions LifeCare - NC

Transitions LifeCare - NC Founded as Hospice of Wake County in 1979. Serves 8 counties including Wake. Oldest, largest and most experienced end-of-life organization in the area. Why?

"Hospice of Wake County, Inc. Becomes Transitions LifeCare April 30, 2014"

For several years Hospice of Wake County has offered more than “hospice” care and has served patients well beyond “Wake County.” Though we changed our name to Transitions LifeCare, our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay. The core brand name “Hospice of Wake County” no longer accurately reflected all we do. We’re not just hospice. And we serve more than Wake County. Our service territory extends well beyond Wake County so we help more patients and families in Durham, Johnston, Franklin, Harnett, Wilson, and Nash counties, eastern Chatham County and in the communities of Chapel Hill and Carrboro in Orange County. Rest assured our expertise, compassion and commitment to helping families facing a serious or terminal illness remain the same. We are still a non-profit organization with a local board of directors. We have not been bought by a large, for-profit corporation. Going forward, Transitions LifeCare is our single brand name and our service lines have corresponding names: Transitions HospiceCare, Transitions Kids, Transitions PalliativeCare, and Transitions GriefCare.

This season, generosity looks like blooming plants and bins full of toys 💚 A heartfelt thank you to Homewood Nursery & Garden Center for the plants and North State Bank for the toy donations supporting Transitions Kids. These gifts help create moments of comfort, joy, and care for the families we serve. Wishing our community a season filled with warmth and kindness. Happy holidays. ❤️🕯️🌲

This Feel Good Friday, we’re grateful to Woodland Terrace Kisco for continuing a tradition that makes a difference. Their annual Stuff the Bus toy drive was delivered to our campus last week, and we can’t wait to share it with our Transitions Kids families this holiday season. Thank you to everyone who helped make this possible! ❤️🎅🕯️

Holiday cheer, handmade decor, and a table full of homemade dishes — all thanks to the Raleigh Garden Club. Each year, they lovingly decorate our Hospice Home and admin building and host a holiday potluck that brings warmth, comfort, and connection to our team. We’re so grateful for their continued support and the heart they pour into everything they do. ❤️🎉🎄

This Giving Tuesday, Lane & Louise Miller are helping us launch our 2025 campaign with a powerful gift — they’ll match the first $15,000 donated. Their belief is simple: supporting a family at the end of life is just as meaningful as celebrating its beginning.

Our site is open early, so help us reach the match by December 2 and ensure compassionate care is there for every family who needs us. ✨💙 Use the link below to make your contribution!

Our Senior Disconnect gathering last week was exactly what we hoped for — a chance to slow down, connect, and bring a little extra warmth to the season. From the blessing-of-the-hands ceremony to crafting holiday stockings and baskets for our Hospice Home families, every moment felt meaningful.

Thank you to everyone who showed up, shared a smile, and helped us spread some comfort heading into the holidays. 💜❄️🕯️
