Transitions LifeCare - NC Founded as Hospice of Wake County in 1979. Serves 8 counties including Wake. Oldest, largest and most experienced end-of-life organization in the area. Why?

For several years Hospice of Wake County has offered more than “hospice” care and has served patients well beyond “Wake County.” Though we changed our name to Transitions LifeCare, our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay. The core brand name “Hospice of Wake County” no longer accurately reflected all we do. We’re not just hospice. And we serve more than Wake County. Our service territory extends well beyond Wake County so we help more patients and families in Durham, Johnston, Franklin, Harnett, Wilson, and Nash counties, eastern Chatham County and in the communities of Chapel Hill and Carrboro in Orange County. Rest assured our expertise, compassion and commitment to helping families facing a serious or terminal illness remain the same. We are still a non-profit organization with a local board of directors. We have not been bought by a large, for-profit corporation. Going forward, Transitions LifeCare is our single brand name and our service lines have corresponding names: Transitions HospiceCare, Transitions Kids, Transitions PalliativeCare, and Transitions GriefCare.

In honor of Black History Month, we recognize Dr. Solomon Carter Fuller, the first African American psychiatrist and a pioneer in Alzheimer’s disease research. Through his groundbreaking work, Fuller helped advance the understanding of Alzheimer’s and brought critical research to the United States.

Despite facing significant racial barriers, Fuller remained dedicated to education, research, and improving care for those living with neurological conditions. His contributions continue to influence the way we understand and support individuals and families affected by Alzheimer’s and other forms of dementia. 📖🧠

We had the privilege of hosting a book reading and signing with Nnenna Freelon for Beneath the Skin of Sorrow. 📖💜✍️

Through story and song, she invited us into an honest conversation about grief, love, and healing — themes that resonate deeply with the work we do every day. She generously stayed to sign books, take photos, and connect with guests, and we were grateful for such a strong turnout.
