Transitions LifeCare - NC

Transitions LifeCare - NC Founded as Hospice of Wake County in 1979. Serves 8 counties including Wake. Oldest, largest and most experienced end-of-life organization in the area. Why?

"Hospice of Wake County, Inc. Becomes Transitions LifeCare April 30, 2014"

For several years Hospice of Wake County has offered more than “hospice” care and has served patients well beyond “Wake County.” Though we changed our name to Transitions LifeCare, our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay. The core brand name “Hospice of Wake County” no longer accurately reflected all we do. We’re not just hospice. And we serve more than Wake County. Our service territory extends well beyond Wake County so we help more patients and families in Durham, Johnston, Franklin, Harnett, Wilson, and Nash counties, eastern Chatham County and in the communities of Chapel Hill and Carrboro in Orange County. Rest assured our expertise, compassion and commitment to helping families facing a serious or terminal illness remain the same. We are still a non-profit organization with a local board of directors. We have not been bought by a large, for-profit corporation. Going forward, Transitions LifeCare is our single brand name and our service lines have corresponding names: Transitions HospiceCare, Transitions Kids, Transitions PalliativeCare, and Transitions GriefCare.

Proud to see our own Dr. Joshua Dowd sharing his expertise at the ANNA Symposium. 💜💭

Hosted by the American Nephrology Nurses Association, this conference brings together nephrology professionals to learn, connect, and stay at the forefront of patient care.

His session, “Getting into the Weeds with Medical Cannabis and Hemp,” tackled a topic we hear about every day from patients and families—bringing clarity, education, and real-world application to an evolving space in symptom management.

We’re grateful for the opportunity to be part of this important conversation. At Transitions LifeCare, we’re committed to meeting patients where they are and exploring thoughtful, individualized approaches to care—and this is a great example of that in action. If you missed it, this is a conversation worth continuing. Thanks so much for having us!

Spring is in full bloom on our campus. 🌸💜

As the days get warmer and longer, here is a simple reminder to pause and notice the beauty around us—sometimes that’s exactly what we need. 🌥️

If you could send one more message… what would you say? 💜

Our Cosmic Post offers a meaningful way to stay connected to loved ones who are no longer physically with us. Write your message, send it skyward, and let the smoke carry your words with love. 💌🕊️

During Social Work Month—and especially today on World Social Work Day—we’re proud to celebrate our Transitions LifeCare social workers and the compassion they bring to patients and families at every opportunity. 💜

Today, we also recognize and thank social workers everywhere for the vital role they play in our communities. Your impact is felt far beyond the moments you serve. Thank you for all you do! ✨🫶

This week, we had the privilege of celebrating Karen Skowronski, Manager of Case Management at WakeMed, as she prepares to retire after over 19 years of service.

Karen’s commitment to the patients and families of our community has made a lasting impact, and we are grateful for the partnership we’ve shared over the years. Congratulations, Karen, and best wishes in retirement!💜🏥

Planning ahead can bring peace of mind—for you and the people who care about you. 💜

We recently hosted an Advance Care Planning workshop at Durham County Library, where community members came together to learn more about documenting their wishes and starting important conversations about future care. 💬

Thank you to the North Carolina License to Give Trust Fund for supporting this program and helping make these workshops possible.

We are so grateful to the CAPTRUST Community Foundation for donating 48 Cheeriodicals boxes to our Transitions Kids program. James Francis Shober made history as the first known African American physician to practice in North Carolina, helping pave the way for generations of compassionate care across our state. His legacy continues to shape the communities we serve every day.

As Black History Month comes to a close, we’re proud to reflect on stories a little closer to home—honoring those who helped build a more inclusive future for healthcare in North Carolina and whose impact continues to be felt today. ❤️🖤💚💛

Caregivers give so much of themselves every day—it means the world when that care is returned. 💙

We are grateful to the Food Lion Caregiver Resource Group for dropping off goodie bags for our staff, patients, and families in honor of National Caregivers Day last Friday. Thank you for honoring them in such a meaningful way. ✨

Our first Senior Disconnect of 2026 was a chance to unplug from the usual pace and reconnect through creativity and conversation. Moments like these remind us that caring for others starts with taking time to connect ourselves. 💜🎨🫟

Thank you to Acclaim at Cary Pointe and Amada Senior Care for partnering with us, and to everyone who made the afternoon so memorable.
