Your child is growing and SHIP needs to hear from you!
The SHIP Study appreciates the time you have taken to allow us to follow-up with you and your child! SHIP depends on your continued participation in order to achieve our goal of better health for children in the future. We are trying to complete follow-ups conducting them by online forms, visits, drop-offs, pick ups, mailings and would like to give you a small packet so you can finish.
*You will still be compensated with gift cards
