Empire Outpatient Services Come as you are. In order to enroll at EOPS you must first make an appointment for an Intake and an Assessment.

This appointment generally takes about two (2) hours. Please call the office to schedule your appointment.

Our Story

In order to enroll at EOPS you must first make an appointment for an Assessment. This appointment generally takes about two (2) hours. We accept all forms of payment. If you have Partnership Healthplan (Medi-Cal) it will cover all of your costs for treatment. If you have private insurance such as Blue Shield of California or Anthem Blue Cross we will help you determine if your provider will cover your services. If you do not have health insurance we will use a sliding fee scale based on your income. No one is denied services because of the inability to pay. Please call our office to schedule an appointment or if you have any questions.
