Offering hospice care, transitions, veterans support, bereavement, and now in-home primary care throughout Central Oregon. Hospice care is designed to focus on quality of life for the terminally ill patient. Our goal is to enable patients to be as comfortable and as free of symptoms and pain as possible, so that they live each day as fully as possible. We believe in treating the patient holistically, honoring their life and their wishes. Our philosophy is to provide support for the patient’s emotional, social, and spiritual needs as well as medical symptoms management as part of treating the whole person. We incorporate a small team approach in providing compassionate personalized care for each patient. Our primary focus is our patients and their families, who we serve with dignity and respect. We believe that our patients have a right to participate in all decisions about their care. We offer suggestions based on sound medical practices but it is our patient’s life and ultimately our patient’s choice as to what will help them on this very personal journey.

Transitions is a free non-medical case management program meant to ensure any adult with a serious illness in Central Oregon has support. Our Transitions Coordinators help clients navigate the challenges which come from having a life-limiting illness.

We know that for teens, sitting in a circle and "talking about feelings" can feel heavy, or just plain awkward. That’s why our specialized team built something different. We’ve designed a retreat that steps outside the traditional therapy room and into a space of action and creativity.

This isn't a lecture; it's an experience. Through expressive art and hands-on activities, we help teens process loss -whether they lost a parent, a friend, or a beloved pet- on their own terms.

A warm meal can really lift your spirits. But for some people in our community, shopping for groceries or cooking has become much harder than before.

If you or someone you care about is spending more time at home because of health or mobility issues, there’s a great resource that can help.

If you’re 65 or older and can’t easily leave home, you may qualify for Meals on Wheels.

At ClearPath Healthcare, we want to help seniors get the support they need. If cooking has become difficult, Meals on Wheels might be just what you’re looking for.

77 days of peace. 77 days of dignity. 77 days of comfort.

Because of your generosity at Festival of Trees, we were able to provide 77 days of full hospice support at no cost to local families when insurance fell short.

You ensured that "ability to pay" never stood in the way of compassionate end-of-life care. To every donor and supporter: you didn't just give money; you gave families the gift of time and peace. We are so grateful.

Camp Sunrise enjoys a reputation as one of the finest children’s grief camps in the nation and the longest-running in Oregon. We host this camp every year because it matters. It matters to the child or teen who has lost someone or something they deeply care about.

We invite you to a special presentation featuring our Bereavement Coordinator, Kat Crawford. With a deep background in counseling, human development, and mindfulness, Kat brings both clinical expertise and a lifelong helper’s heart to this delicate topic.

The short answer to two common questions we receive is yes.

The services hospice provides differ from those provided by the care team in a senior living facility and by hired caregivers. Our hospice is accustomed to supporting patients in senior living facilities, working with the facility's staff to ensure your loved one receives the best possible care. Our hospice is also accustomed to working alongside home care agencies to support your loved one in the home. Hospice is a different service from caregiving, and quite often, our patients need both.

Each year, the Festival of Trees features a special Camp Sunrise tree, where every dollar raised goes directly to providing grieving kids with a place to heal, connect, and grow.

This year, because of YOU, that tree sold for an incredible $9,000! Plus, it took home the People's Choice Award!

A huge shout-out to our Bereavement Coordinator, Kat (pictured in front of the tree), who designed and decorated this masterpiece. Her heart for our campers shines through every branch.

Thanks to your generosity, more children will experience a weekend of hope and healing at Camp Sunrise this year. Thank you for ensuring that no child has to navigate grief alone.
