At ClearPath Healthcare, our Transitions program may be small, but the numbers tell a story of dedication, grit, and life-changing results right here in Central Oregon.

In the last few months, our team has been working to ensure no one in our community is left behind. Here is a look at the "ClearPath Effect":

$58,000: Back pay secured for our local veterans from the VA in just 6 months.

$24,500: Saved for our low-income clients by navigating the Medicaid system.

1,500: Miles driven every single month to meet our clients where they are.

79: The average number of people enrolled in our Transitions Program.

2: The number of dedicated Transitions Coordinators making this all happen.

It doesn’t take a massive department to move mountains—it takes a team that cares. Whether it's securing earned benefits for veterans or ensuring healthcare access for those who need it most, we are in the business of changing lives.

Did you know that ClearPath Healthcare (formerly Hospice of Redmond) helped write the blueprint for hospice care in America?

In 1979, we were selected by the Federal Government as one of only 26 "demonstration programs" nationwide. We were tasked with a massive mission: to help determine what quality end-of-life care should look like and how to make it sustainable for everyone.

We started small but mighty:
• An all-volunteer agency fueled by pure heart.
• Serving just six patients per month.
• Helping define the very standards of care used across the U.S. today.

Fast-forward 45 years, and that same pioneering spirit drives us at ClearPath Healthcare. We’ve grown from those first six patients to serving all of Central Oregon, but our commitment to quality, compassionate care remains unchanged.

The best medicine for the soul often comes with fur and a purr (or bark). ❤️ Pets offer unconditional love, lowering stress and bringing comfort, especially during challenging times. We see the healing power of companionship every day as we visit our patients. If you don't have a pet, we have pet therapy dogs that can be brought in to provide some furry love!

How many friends or family members will it take to care for me at home while I am on hospice?

There’s no set number. One of the first things we will do is prepare your individualized Care Plan that will, among other things, address the amount of caregiving you need. Your Care Plan will be routinely reviewed by your Care Team and amended as your needs change. ClearPath Healthcare staff is on call 24/7 for your support.
