Our story

Revive Health Senior Care is a new type of Skilled Nursing and Rehabilitation Management Company. The founders are Zachary and Chelsey Gray, a husband-and-wife team, whose unique backgrounds encouraged them to look at the way healthcare is being provided to Seniors and the disabled, and think, there has to be a better way.

On February 1st, 2019, Zach and Chelsey purchased Alta Skilled Nursing and Rehabilitation Center, a 174-bed skilled nursing facility (SNF), which staffs over 100 employees providing the best care for our patients.
