Medicare Certified Shrine Home Health agency is looking for an experienced Home Health Care Community Liaison with an established following with physicians, facilities, and other home health referral sources in the Dallas-Fort Worth metroplex. Ideal situation for an experienced marketer. Perfect situation for outstanding rep currently visiting physicians, assisted living facilities, skilled nursing, hospital discharge planners, etc.
Duties:
• Operations: Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns. Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager. Monitor Home Health Aide supervisory visits.
• Patient Care: Work in the field with staff and patients as needed.
• Customer Service: Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
• Minimize Patient Risk: Ensure Start of Care compliance and quality and safe delivery of home health services. Focus on Outcome Monitors as assigned. Implement quality improvement programs in the field.
