Treating the source, not the symptoms!

Over the decades your strength fades, muscle mass declines, as your aerobic capacity tanks. Tendons and connective tissues lose substance, stiffness, and resilience. For years the body compensated... quietly. Then one day a knee hurts during a run to get the train, or shoulder aches reaching overhead, or a back tightens lifting something simple.

At that point the story usually becomes more about structural damage. An MRI gets ordered. Welcome to high-tech, low-medicine. And the MRI almost always finds something. A meniscus tear. A rotator cuff tear. A disc bulge. Why? Because by midlife these findings are extremely common — even in people with no pain at all. If you have a tear in one shoulder, image the other shoulder... you probably have the same tear there. But I digress.

Once the scan appears, the narrative changes. The image becomes the diagnosis. Now the patient believes something is broken, and the focus often shifts to fixing what the MRI shows.

What often gets lost in this is the reason the symptoms appeared in the first place. Many so-called “atraumatic” orthopedic complaints are not purely mechanical failures. They are the moment when reduced strength, declining tissue capacity, and sometimes broader metabolic health issues finally reach a tipping point. Our tissues change over the decades... get over it.

In other words, the MRI didn’t create the problem. Well... it sort of did in this scenario. But all the MRI showed was something that was already there.... because of your age, lifestyle, health and so on.

The real driver of symptoms is often loss of physiologic reserve. Less muscle. Less tendon or aerobic resilience. Less tolerance for load, etc.

Once the MRI enters the picture, the risk becomes overtreatment. This is probably the number one reason people have surgery. When in many cases the most powerful intervention was never the scan or the procedure.

Too often, we see patients after a small problem has turned into a big one.

That nagging shoulder ache… the tightness in your back… the knee that “only hurts sometimes”… these are your body’s early warning signs.

Physical therapy isn’t just for injuries after they happen—it’s one of the best ways to prevent them from getting worse.

When you come in early, we can:
✔ Identify the root cause
✔ Correct movement patterns
✔ Reduce pain before it becomes chronic
✔ Keep you active and doing what you love

Waiting often means longer recovery, more discomfort, and more limitations.

World Day for Physical Activity 2026

We all know the benefits of physical activity, but many of us are not meeting the physical activity recommendations. The WHO reports that 1 in 4 adults, and 4 in 5 adolescents, do not do enough physical activity. Older adults, people living with disability, and women and girls are also less likely to be active, further widening health inequalities.

The physical therapists at Orthopedic Physical Therapy evaluate and treat your whole body and look for the source of the pain. Treating the symptoms does not usually bring long lasting results 

Pain is a signal, not a diagnosis. Your knee may be saying, “I’m weak,” but it’s often heard as, “I’m broken.”

Most people I see are scared. They stop running or going to the gym not because the pain is severe, but they’re worried they might make it worse.

That’s why it’s critical to ask: what brought you in, the pain itself, or the worry about what it means?
The answer shapes everything that follows.

In many cases, these aches and pains improve within a few weeks, and most people can expect a full return to their usual activities.
After an exam and sometimes an X-ray we may consider further imaging, but often we don’t need to.

When we don’t distinguish between pain and fear, it can lead to overdiagnosis and overtreatment. For some clinicians (not all), the presence of any abnormality on an MRI can lower the threshold for recommending surgery.

I’m actually writing a book about this exact issue, it’s far more complex than most people realize. Especially when you consider that an estimated 30–50% of commonly performed orthopedic surgeries may not be necessary.
