The event will be held on November 13, 2026 at 7 PM at the Dewey Gottwald Center in Richmond.

Uncork Hope is an evening dedicated to raising funds that directly support children with disabilities and the families we serve across Virginia. We are grateful for the continued commitment of our community and look forward to gathering together again this fall.

The Virginia Down Syndrome Association will host its 12th Education & Transition Conference on March 14, 2026 at J. Sargeant Reynolds Parham Campus.

The event includes three conference tracks and a keynote presentation by David Egan and is designed for educators, caregivers, and young adults with intellectual & developmental disabilities. Scholarships are available.

Planning for your child’s future can feel daunting, but you don’t have to do it alone. MHP's Optimal Futures Program will work with you to navigate the world of disability services and create a clear path for you and your child to follow into adulthood.

MHP's Optimal Futures Program starts working with students the year they turn 14 and continues four to five years after exiting high school. Students will be supported with developing and following transition plans that empower them to achieve adulthood goals. The Optimal Futures team will support by identifying resources, linking to services, helping to build relationships, and staying in regular mentorship contact with the students to ensure they realize their *optimal future*. Services are free of charge and a financial pre-screening is required. Services are limited at this time to the Greater Metro Richmond area. Please contact us to determine if your child qualifies!

Medical Home Plus: Your Child is our Mission

Medical Home Plus (MHP) was created as an extension to Richmond Pediatric Associates in 2001. In 2003, MHP became incorporated as its own entity. Founded on the principle that every child deserves a Medical Home, Dr. Colleen Kraft, Medical Director, established MHP as an organization that employs parents successfully navigating the system of care for their children. This innovative staffing model has allowed MHP to forge a unique connection with families in need based on experience, empathy and trust.
