Chronic Kidney Disease affects 1 in 7 adults. Many of those persons are not aware that they have CKD until their illness is considered in an advanced stage. More than 800,000 individuals are diagnosed with End-Stage Kidney Disease per year in the US.

Admitting to Hospice Care under a renal disease diagnosis does not always mean giving up; oftentimes, individuals who receive Hospice Care see an improvement in condition due to the close monitoring of symptoms through the 24/7 available care. Individuals who might be considered for admission are those who: are not or are unable to receive a kidney transplant/dialysis, have fluid overload that does not respond to treatment, and possess symptoms of/are diagnosed with end-stage renal failure.

** These are guidelines for determining Hospice eligibility. Some patients may not meet the criteria but may still qualify for services due to comorbidities or rapid functional decline. Contact our office for further information. **

Join our compassionate healthcare team, in San Antonio, as a Hospice Care Coordinator, where your dedication will directly impact the quality of life for patients and their families during challenging times. In this vital role, you will coordinate referrals with patient/family meetings to explain the hospice program and obtain needed medical records to ensure seamless delivery of hospice services. Your organizational skills and empathetic approach will help create a comforting environment for those in need of end-of-life care.

Duties:
• Serve as the primary point of contact for patients, families, and healthcare providers to coordinate hospice services effectively.
• Educate patients and families about hospice services, and available resources to promote understanding and comfort.
• Collaborate with interdisciplinary teams including nurses, social workers, chaplains, and physicians to deliver holistic care.

Requirements:
• Proven experience in a medical office setting or healthcare environment; hospice or palliative care experience is a plus.
• Excellent organizational skills with the ability to manage multiple priorities efficiently.
• Compassionate communication skills to support patients’ emotional well-being and assist families through difficult decisions.
• Ability to work collaboratively within a multidisciplinary team while maintaining attention to detail.

Honoring Our Unaccompanied Veterans

Please join us as we honor our Unaccompanied Veterans—men and women who faithfully served our nation and are laid to rest without family present at their funeral services. These heroes deserve to be remembered, and it is our collective responsibility to ensure they are never alone.

Fort Sam Houston National Cemetery, in partnership with the Bexar County Department of Military and Veterans Services, will host a memorial service in tribute to these veterans. Full military honors will be rendered by the Wednesday Squad, Memorial Services Detachment.

Time: 9:00 AM
Location: Fort Sam Houston National Cemetery – Assembly Area 1

Community members are encouraged to attend and stand with us in honoring those who served with honor and dignity.

Our Annual Christmas Meal 4 Vets campaign is just 3 days away. For three years now community vendors have come together to feed Veteran Families in Nueces County. This year we had some very generous donors that allowed us to purchase larger hams & a handful of Christmas Gifts for the younger Veteran’s family members.

This year’s shop took place at H-E-B of Beeville. Thank you to employee Wyatt for being king enough to let us take the rolls still in the shipping boxes for ease of transport. Employees Amanda, Valerie and Ray for going above and beyond in getting us checked out and on our way at lightning speed.

We can’t wait to share more photos with you all next week! 🇺🇸🎄
