Important Public and Legal Statement Regarding Medical Records
Notification of Medical Records Breach
This notice is to inform all Davis Family Health patients of a recent breach concerning your medical records. It has come to my attention that records dated before April 1, 2025, are now in the possession of MedCare Health LTD and 1st Convenient Care, both located at 1212 Currency Ct, Rochelle, IL 61068.
Currently, MedCare Health has locked Davis Family Health out of the electronic program that stores your medical records. This situation has been verified by a written transcript from Practice Fusion, the charting system provider utilized by our clinic.
What Does This Mean for You?
If you need access to your medical records, you will now have to contact MedCare Health directly. Alternatively, Davis Family Health can submit a form to MedCare Health to request summaries of your records. Please note, it is uncertain whether these requests will be honored or processed. I am actively pursuing legal and professional action to address this issue.
Background of the Situation
Prior to August 26, 2025, Davis Family Health and MedCare Health shared access to the Practice Fusion program, which Davis Family Health financed. Given Dr. Cassandra Headon’s commitment to contractual obligations, there was no concern that patients’ records would be compromised. However, with Davis Family Health now locked out of the charting program, your records—including addresses, diagnoses, and phone numbers—are no longer secure.
If you require your records for disability claims or further treatments, please be aware that MedCare Health currently controls this information. Some of you may recall receiving texts or calls from MedCare Health encouraging you to continue care with their new provider.
Clinic Relocation and Staffing Changes
Davis Family Health vacated the MedCare building in early September as our clinic, along with Trim IV Infusions, expanded operations. Additionally, the MedCare building was approaching its occupancy limit as specified by the Rochelle City Building codes—a fact evident on Wednesdays and Fridays during Social Security Clinics.
Upon learning of our departure, MedCare Health hired a new practitioner, as indicated by business licenses filed on September 2 and a public announcement made by MedCare on September 5. This further contributed to the building reaching maximum occupancy.
Those of you who have tried to schedule appointments with Davis Family Health since September 10, 2025, may have been told, “she is no longer here, no forwarding address or forwarding phone number.” This response mirrors what occurred when Patricia Adamski and Dr. Michael Rivera left the MedCare clinic.
Clinic Operations and Patient Choice
Mendota Davis Family Health has been serving patients since October 1, 2021. MedCare Health acknowledged our opening by sending flowers, and we continue to receive records requests at the Davis Family Health fax number. Both of our clinic locations are well known in the community.
I want to emphasize that patients have the right to choose their healthcare provider. While I hope you continue your care with Davis Family Health, you are free to seek care from the new provider at MedCare Health if you so choose.
