At SMB Disability Solutions, we believe that every person has unique qualities, abilities, and perspectives. Our mission is to guide people toward that potential and help them set the stage to fulfill their goals and dreams. SMB was founded in 2005 as a social service organization that offers a full range of care services to adults 18 and older with brain injuries, mental illness, and other disabilities. Based in Rochester, Minnesota and serving the greater Rochester and Olmsted County area, SMB focuses on the growth of each person in five key areas of wellness: spiritual, mental, emotional, physical, and social. SMB provides the following services:
• Adult Residential Services (Permanent, Respite, and Emergency beds)
• Day Support Services with Transportation
• Individualized Home Supports with Training
Residential
For individuals in need of a place to live, we offer housing and 24-hour residential care through our Residential Support Services (RSS) . RSS consists of 7 locations throughout Rochester. We work closely with potential residents to find the right home and to develop individualized life goals and skill plans to guide our residents to achieve their goals. RSS also provides out-of-home respite care to ensure caregivers are able to take time for themselves, as well as short-term emergency stays for individuals who need a safe place to get back on their feet. Day Support Services
In addition to housing, we offer a range of day programming through our Day Support Services (DSS). DSS consists of two program sites with an urban and rural focus. Our urban Day Program offers endless opportunity for community building, socialization, crafts, exercise, and creative expression. Our rural Farm Program offers therapeutic activity, socialization, education, skill building, and community engagement through a 2.5-acre vegetable farm and nature area. These programs are open during the weekdays and provide an alternative platform for skill development and life engagement for those not able or interested in working, who are retired, or who want to fill out part-time work schedules with other activities. Our Individualized Home Supports (IHS) serves individuals who live independently but need additional support with developing living skills or help achieving their personal goals. IHS offers participants assistance with developing self-care and life development skills they need to succeed and be safe and comfortable living by themselves. With one-on-one care and attention, participants are able to focus on the specific challenges they face, such as budgeting, meal planning, schedule management, etc., while receiving guidance to implement solutions in a way that supports independence.
