For 17+ years, Dr. Natalya Danilyants, MD, FACOG, has been a board-certified gynecologic surgeon specializing in minimally invasive treatments for complex GYN conditions like adenomyosis, fibroids, and endometriosis. After 17 years of treating women with endometriosis, one of the most common questions I hear is:
“How can my MRI or ultrasound be ‘normal’ if I’m still in pain?”

Here’s the truth: Most endometriosis is often missed on imaging. The implants are usually too small or hidden for ultrasound or MRI to detect. These tests can find cysts or advanced stages of the disease, but they miss what causes daily pain for at least 1 in 10 women.

That’s why it’s so important to see a specialist who understands this condition. A surgical evaluation through laparoscopy is still the gold standard for seeing and treating endometriosis—through excision.

Myth: A hysterectomy always causes menopause.
Fact: Menopause happens after a hysterectomy ONLY if the ovaries are removed for a medical reason. Most of my hysterectomies preserve the ovaries (and often the cervix)—so hormones and sexual function are maintained.
For women not seeking future fertility, I perform an advanced minimally invasive hysterectomy using CIGC’s DualPortGYN technique: two tiny, papercut-length incisions. It’s outpatient (home the same day), and recovery is days, not weeks.

If you do want to keep the option to get pregnant, that’s a different path: uterus-preserving fibroid surgery like CIGC’s LAAM myomectomy (not a hysterectomy).

Adenomyosis is far more common than most people realize — yet it’s often misdiagnosed or overlooked. It's often mistaken for endometriosis, but they’re different. Endometriosis happens when the endometrial lining grows outside the uterus. Adenomyosis occurs when that same lining grows into the muscle of the uterus. Once inside the muscle, the glands respond to each cycle — leading to severe pain and heavy bleeding.

Too many women are dismissed or passed from doctor to doctor for years before finally reaching a specialist who understands what’s going on.

For more than 15 years, I’ve been diagnosing and treating adenomyosis, and I know how disruptive it can be. Common symptoms include:
• Heavy bleeding
• Severe, labor-like cramps
• Pain that lasts through much of the month

Because it doesn’t always show up on imaging, diagnosis is usually clinical, with MRI being more helpful in some cases.

Treatment depends on your fertility goals. For women with focal adenomyosis, fertility-sparing surgery may sometimes be an option. A hysterectomy remains the only cure — but contrary to common belief, it does not automatically mean removal of your ovaries. If your ovaries are preserved, you will not go straight into menopause. (I’ll share more about this in another video.)

❗So you’ve discovered small fibroids—now what?❗They should never be ignored. As long as you’re producing estrogen, fibroids will grow. Even tiny ones can cause infertility or force more extensive surgery later if they aren’t monitored.

She’s only 18.
We don’t usually operate on patients this young—but today, I had to remove large fibroids from her uterus.

Fibroids are extremely common, especially among black women, 90% of whom will develop them in their lifetime. Left unchecked, they can grow quickly and threaten fertility and cause major complications later on—sometimes long before you expect it.

This is why medical surveillance matters (regular ultrasounds):

• Early detection means less complex surgery and better outcomes
• Regular check-ups catch growth before it’s too late
