In August, we planted a seed. We launched our collective with a simple mission: to ensure no family in Southeast New Mexico had to walk the thresholds of birth or loss alone. We were a small circle of Keepers, protecting the "Arrival" of the next generation. ​Now, it is March-a season of renewal. ​As we expand our reach into local clinics and hospitals, our mission is evolving. We are transitioning from a collective of individuals into the Kihci Ancestral Ways Institute. Our focus is shifting toward a future as a non-profit dedicated to professional training and community education. We are bridging the gap between clinical safety and ancestral wisdom, anchored by our AWAKE pillars. ​We aren't just doulas anymore. We are training the next generation of Threshold Guardians. We will use this time to plant the seeds and hope for growth from our community. The bridge is being built. The school is open. The arrival is here. Stay tuned for the launch of our first cohort!

Getting a diagnosis for PMDD often feels like finally being handed the legend to a map you’ve been trying to read in the dark for years. It isn’t just "bad PMS"-it is a legitimate neuroendocrine disorder where the brain overreacts to the normal hormonal shifts of the menstrual cycle.
​When you add neurodivergence into the mix, that sensitivity is amplified. For those of us with ADHD or Autism, our nervous systems are already processing the world at a higher volume; PMDD turns that volume up until the speakers start to blow.

​The Reality of the "Luteal Storm"?
​PMDD happens during the Luteal Phase-the time between ovulation and the start of your period. While most people experience some shift, for those with PMDD, the drop in estrogen and the rise in progesterone products (specifically allopregnanolone) triggers a paradoxical reaction in the brain’s GABA receptors. Instead of feeling calm, the brain enters a state of high-alert, rage, or deep despair

​Common Symptoms Include:
-​The Emotional Crash: Sudden, intense feelings of hopelessness, rejection sensitivity (RSD), or "luteal rage."
-​The Cognitive Fog: A significant spike in executive dysfunction-forgetting appointments, losing items, and feeling unable to "mask" anymore.
-​Sensory Allodynia: Physical pain from textures or sounds that are usually fine.

​The Neurodivergent Intersection
​Research from 2024 and 2025 has increasingly shown that people with ADHD and Autism are significantly more likely to experience PMDD.
-​The Dopamine Drop: Estrogen acts as a "helper" for dopamine. When estrogen falls, dopamine levels in the brain plummet. For an ADHD brain, this makes medications feel less effective and basic tasks feel impossible.
-Sensory Overload: The hormonal shift lowers our sensory threshold, leading to quicker meltdowns or shut-downs.

​As a doula, our role is to provide informational and emotional support, helping you navigate these options with your clinical providers.

1) ​Western Medical Pathways
-​SSRI "Pulse Dosing": For PMDD, certain SSRIs can be taken only during the luteal phase. Unlike treating standard depression, they often work within hours or days to stabilize the brain's reaction to hormones.
-​Hormonal Suppression: Some find relief by using specific birth controls to "level out" the cycle, preventing the sharp drops that trigger the brain.

2) ​Evidence-Based Nurturing (Holistic)
​If you prefer non-pharmaceutical tools or want to add to your toolkit, these have strong clinical backing (You should ALWAYS consult with your medical provider before trying something new for treatment):
-​Somatic Movement: High-intensity exercise can actually worsen PMDD symptoms by spiking cortisol. Instead, look into vagus nerve regulation-gentle stretching, cold water face immersion, or somatic "shaking" to help the body process the "luteal rage" physically.
-Sleep Hygiene: Focus on your bedtime routine to optimize melatonin. Reduce white light, wind down hours before bed, meditation before bed, natural waking without alarms if possible, first 20 minutes being awake is outside in the sun when possible.
-Tall to your provider about calcium to support your parathyroid. Magnesium can help with GABA receptors, which can ease anxiety and irritability.

3) ​Nurturing Your "Luteal Buffer"
​Healing PMDD is about accommodation, not just treatment.
-​Lower the Demand: Identify your "Hell Week" on the calendar and proactively cancel high-masking social events. Give yourself permission to do the bare minimum.
-​The 24-Hour Rule: Make a pact that you will not make major life decisions (breaking up, quitting, or big purchases) during the luteal phase.
-​Sensory Sanctuary: Switch to your "safe foods" and softest clothes a few days before your symptoms usually start.

​This diagnosis is not a life sentence; it’s a tool for self-compassion. You are learning how to parent your own nervous system through a monthly storm.

Imagine it is 1930. A lantern is lit in a window in the Georgia pines. Inside, a Granny Midwife is rhythmically massaging a mother’s abdomen. Her hands are clean, her spirit is calm, and she is following a protocol of hygiene and herbs passed down through generations.

She is the "Watchful Eye."

​But outside that house, the world is changing.

​In sterile city offices, men are signing the Sheppard-Towner Act into law. They are drafting pamphlets that call this woman, this expert who has delivered 3,000 babies without a single loss, "unclean." They are moving birth into hospitals where, at that very moment, doctors are walking from autopsy tables to delivery beds with unwashed hands, carrying the "invisible death" of sepsis into the wombs of mothers.

​The medical establishment didn't just want the practice; they wanted the market. And to take it, they had to colonize the very idea of safety. They used the law like a scalpel, cutting the Granny Midwife out of the community and replacing her with a system that didn't know the mother's name, her lineage, or her soul.

​For decades, the thread was thin. The hospitals became the only "legal" choice, and the results of that colonization are written in the forensic data of 2026: Black mothers dying at three times the rate of their peers. This isn't a medical mystery; it’s a historical consequence. It is the result of removing the guardians.

​The Modern Reclamation;
​But here is where the story shifts.
​When our students sit down to learn, they aren't just memorizing biology or "operations." They are sitting at the feet of those Grandmothers. In our collective, every doula and birthkeeper begins their journey by unlearning the "dirty" narrative.

​We teach them the forensics of the sepsis era so they understand that "professional" hasn't always meant "safe."

We teach them the law so they can advocate against the modern "Sheppard-Towner" hurdles. We teach them that their hands carry the same ancestral weight as the lanterns in the 1930s.

​Why Do We Study This?
​We don't study this history to stay in the past. We study it so that when our doulas walk into a hospital room today, they aren't just "support persons." They are witnesses. They are the shield that the colonization of birth tried to break.
​Our students learn this history because you cannot fix a system until you understand how it was intentionally broken. We are the reclamation. We are the thread, finally made whole again.

A 2023 meta-analysis showed how doula support improves maternal health and birth outcomes.

This comprehensive review of data from 16 peer-reviewed studies confirmed what many of us have long observed: doula support matters and it is associated with improved maternal and infant health outcomes.

These studies found that doula support was associated with:

✅️ More Positive Lactation Outcomes
✅️ Shorter Labor and Less Pain Medication Use
