We’re beyond proud of Dr Sarah Rivas DNP, AGPCNP-BC, a former Johns Hopkins University intern with Senior Care Clinic Medical House Calls, now board certified, who’s starting her new role as a Neurosurgery Advanced Practice Provider at the Swedish Neuroscience Institute in Seattle, WA! 🧠💙

Skilled wound care brought to the comfort of home by board-certified NP...

With a career rooted in compassion and clinical excellence, Mallorie Hogan, MSN, FNP-BC, WCC, has built her professional life around advancing patient care across diverse healthcare settings. From her early service as a volunteer in the UC Davis Emergency Department to her years as a Charge RN at a skilled nursing facillity, Mallorie demonstrated a steady commitment to both patient safety and team leadership. Her ability to balance direct bedside care with supervisory responsibilities—overseeing LVNs and CNAs while ensuring timely interventions—helped shape her into a practitioner who values both the human and technical aspects of medicine.

Mallorie’s recent roles as Nurse Practitioner, Clinical Nurse Manager, and Assistant Medical Director at a home health agency further highlight her leadership in home health and community-based care. She has managed responsibilities ranging from vaccine coordination during the pandemic to supervising COVID-19 testing programs, reflecting both adaptability and public health commitment. I first met CarePoint Healthcare Hospice co-founder, Owy Holmquist when I was just starting my practice—ten patients, a borrowed office space, and the quiet uncertainty of building something from the ground up.

She was new to marketing then, too, just finding her own footing. But even in those early days, she did something rare: she showed up. Not once or twice, not only when a referral was needed—every week, without fail. Her presence wasn’t transactional; it was intentional. She made time.

That consistency became her signature. For over a decade, she built her marketing career not just on strategy, but on trust earned through presence. Then, she transitioned into owning and operating care homes, bringing that same dependability to the world of direct caregiving. She had studied nursing—two full years—before stepping away for family. But the heart of a caregiver never left her.

Now, as the co-founder of a clinician-owned and operated hospice agency, she leads with the same steady poise. Her agency partners seamlessly with Senior Care Clinic Medical House Calls (a home-based palliative and primary care practice), forming a continuum of care rooted in relationship and reliability.

Because of that consistency—her habit of showing up, listening, following through—those she once visited as “clients” now consider her family. I do too. She’s no longer just a colleague or collaborator. She’s part of the foundation. Part of the reason we’re still here. Part of the reason we keep going.
