Crowdfund Project for Pediatric concurrent care program at UC Davis Health!
The Pediatric concurrent care program at UC Davis Health is a supportive service for children with life-limiting or serious illnesses. This program allows children to continue receiving curative or palliative treatments while also receiving end of life care and support from the hospice team. The care is provided through a collaboration between home health and hospice nurses to ensure coordinated, compassionate care. Our program provides a family-centered approach, through an interdisciplinary team of nurses, social workers, chaplains, rehabilitation therapists, and bereavement specialists. The goal of our program is to walk alongside families through a profoundly difficult time.
Our pediatric concurrent care program was selected to participate in the Children’s Hospital Crowdfund project. The campaign runs from September 2-19 and our fundraising goal is $5000.
Donations to this project will fund meaningful legacy-building activities such as heartbeat recordings, memory books and digital storytelling. Donations will also help to fund nutritional support and funeral expenses for families who are in need. Every gift makes a difference in bringing care, connection, and comfort to a child and their family. Donations are also tax deductible.
